救命士: 目撃者がいないんです。

■カーターは得々と診断を述べる。

カーター: 患者は右下肢に激痛を訴えています。
結腸腫瘍の病歴を持ち、飛行機に四時間乗りました。
Patient is experiencing pain in the right lower extremity.

Recent history shows colon cancer and a 4-hour plane flight.

ベントン: フーっ。

カーター: 右ふくらはぎに触れると痛みが走り、
左より四センチも太くなっていることが検査により判明。
可能性として蜂窩織炎、損傷、血腫、ベーカー嚢腫に深部静脈血栓症です。
Upon examination patient's calf was found to be tender and

four cm larger than the unaffected side.

Differential diagnosis includes cellutis, hematoma and deep

vein thrombosis.

ベントン: そうか。それでどうする?
Okay. So what would you do?
カーター: 血算に生化学、凝固機能、足の造影スキャンを提案します。
I siggest a CBC, Cham-7, coag study and a duplex scan of

leg.
ベントン: そうか。また来ます。
All right. Bye now.

カーター: ウィルヒョウ三徴のうち二つが見られるので、

もしスキャンでも特定できない場合には、静脈造影を行います。
In addition given that two elements of Virchows's Triad are

present I feel a venogram is indicated if the scan is

inconclusive.

ベントン: 続けろ。
Proceed.

カーター: 僕の診断に賛成ですか?
So, you concur with my recommodation?

ベントン: 余計なことを言うな。
I said proceed.

■仮眠室深い眠りのいびきと
ポケベルのビープ音が同居する暗闇。
ドワが開いて通路の光が差し込む。

ベントン: くわぁ～たー !　よだれかけをしたほうがいいな。
Carter! Man, somebody get a drool bucket.
おい、起きろ。起きろ、起きろ、サボるな !
五分もポケベルを鳴らしてるんだぞ。

カーター: ああ、スイマセン。

ベントン: これは必ず枕の下に入れとけ。
そうすりゃ、聞こえるんだ。

二号室に頭部に裂傷を負った不潔な男のホームレスがいる。
傷口を洗浄して縫合しろ。

ベントン: What is it?
リィディア: すぐ急患がきますよ。腸穿孔らしいです。
EMTs are on their way. They think it's a ruptured bowel.

■通路　キャリアに運ばれながらうめき声を上げる患者。
そばにベントンが駆け寄る。

ベントン: 脈拍は?
What's the pulse?

救命士: １４０です。血圧は６０の４０。
Pulse is 140. Pressure 60 over 40.

点滴はもう1500cc入れました。腹部が硬直してます。
We've been pumping in fluids. He's had 1500cc's.
Abdomen's rigid, extremely tender.
ここで血栓症の疑いがあると言われたそうです。
Says he was here today with a thrombosis.

ベントン: :血算、血液型とクロスマッチ６単位、
ニトロを使う。オペ室に準備させろ。試験開腹が必要だ。
Let's get a CBC. Type and cross six units.
Let's drop an NG. Call the O.R.,
tell them to prep for an exploratory lap.

ジェリー: Yes.

ベントン: こっちだ。急げ。手際よくやれ。
サンハーストさん。心配ないですよ。
こういうことはよくあるんですから。
