Double eyelids are not achieved by incision alone.
The incision must allow the power of the levator palpebrae superioris muscle to reach the skin and orbicularis oculi muscle. Sometimes, in many cases, the levator aponeurosis is unnecessarily advanced forward. Unnecessary advancement allows the levator aponeurosis to reach the skin, resulting in unnecessary eyelid opening. This can result in a scary-looking appearance or excessive central eyelid opening. To achieve double eyelids without unnecessary advancement, a tendon graft is simply used to bridge the gap from the levator aponeurosis to the skin.

二重は切開だけで成立しません。 上眼瞼挙筋の力が皮膚と眼輪筋に届くように行う必要があります。
まれに、上眼瞼挙筋腱膜を不必要に前転している症例があります。
不必要な前転は、挙筋腱膜が皮膚に達し二重になりますが、
結果として不必要にまぶたが開いてしまいます。
その結果、見た目が不自然になったり、まぶたの中央が過度に開いたりすることがあります。
不必要な前転を避けて二重まぶたを成立させるためには、
腱移植を用いて上眼瞼挙筋腱膜の断端から皮膚までの隙間を埋めるだけで済みます。
