私のケアマネージャー
大きな手術を受けたり、しばらくの間入院が必要になったり、ある病気のために家から出られなくなったりしたら、まずケアマネージャーを探す必要がある。ケアマネジャーとは、皆さんの治療を最初から最後までコーディネートしてくれる人のことである。
まず最初に、市役所の福祉課に連絡しなければならない。そこでケアマネジャーとの面会方法を教えてもらい、自分の状況を説明することができる。私のケアマネジャーがどのように選ばれたのか、私は意識がなかったので詳しくは説明できないが、彼が私のために、つまり私のケースに対してどのように働いているのかを説明しよう。一般的に、医療制度は標準的な方法で機能するが、それぞれのケースは異なる。ですから、これからお話しするのは私の経験である。脳出血はいつでも起こりうるので、最初の治療はどの病院に入院したり、その病院での治療の可能性があったり、必要であればその病院からの転院の可能性があったりする事もある。
ケアマネジャーの事に戻る。彼は私の治療を調整したり、情報を持っていたり、私の治療に関わる医師、看護師、理学療法士、ヘルパー等を知っている。私が服用している薬もすべて知っている。必要であれば、リハビリセンター、病院、新しい医師、新しい理学療法士等もを提案してくれる。私の承認があれば、すべての医療専門家が私の治療に関するすべての情報を知っている。連携がとれている方がいいと思う。ケアマネジャーは、私の治療情報が共有され、それで私の治療がとても迅速に流れるようにしてくれる。つまり、私の状態に関する情報システムを作ってくれているのである。ケアマネジャーは私自身よりも私の健康状態を知っている。
私の場合、担当医である名古屋市の大同病院で定期的に診察を受けている。主治医であり、私を受け入れて脳を手術してくれた先生だから、彼と相談することになっている。そのため、毎月病院に行くと主治医に状況を相談し、他に解決しなければならないことがあれば、別の専門医に診てもらうことにしている。
ケアマネジャーは月に一度、私を訪ねてきてもらう。治療はどうなっているのか、私の健康状態はどうなのか、新しい専門家は私とうまくやっているのか、などといった質問をしてくれる。また、私が主治医に伝えるべき質問も提案してくれる。ケアマネジャーは医師ではないので、担当の看護師が毎月私を訪問し、健康状態全般について質問してくれる。緊急の治療が必要な場合は、すぐに医師に連絡してくれる。
彼はとても面白く楽しい人で、皆さんは彼を「大佐」と呼んでいる。

My Care Manager
If you undergo a big surgery, need to stay in hospital for a while, or are unable to leave home because of certain illness conditions, you need to look for a care manager in the first place. A care manager is the person who is supposed to coordinate all your treatment, from beginning to end.
First things first. You have to contact the Welfare Division in the City Hall. There they will be able to inform you how to meet a care manager and you can explain your situation. I'm not going to give you details on how my care manager was chosen because I was unconscious but, I'll try to describe how he works for me, I mean, for my case. Generally, the health system works in a standard way but each case is different. So what I'm going to tell you is my experience. A stroke can happen at any time so your initial treatment will depend on which hospital you were admitted to, treatment possibilities in that hospital, transfer possibilities from that hospital if needed, and so on.
I am going back to my care manager. He coordinates my treatment, has information, or knows the doctors, nurses, physiotherapists, helpers, etc, involved in my treatment. He knows all the medication I am taking. He suggests rehabilitation centers, hospitals, new doctors, new physiotherapists, etc, if needed. With my approval, all health professionals know all the information concerning my treatment. It's better if they work coordinated, I think. The care manager makes sure that my treatment information is shared and it flows very quickly, I must say. In other words, he creates an information system about my condition. They know more about me than myself.
In my case, I have regular consultation sessions at Daido Hospital in Nagoya City the doctor responsible for my case. He's the main doctor (I'll call him " chief doctor" from now on.) and I'm supposed to have consultations with him because he was the one to receive me and operate on my brain. So when I go to the hospital monthly, I discuss my situation with him and if there is something else to be solved, he sends me to another specialist.
My care manager visits me once a month. He asks questions like how the treatment is going, how my general health state is, if a new professional is doing well with me, and so on. He also proposes questions to me that I may take to my chief doctor. Since my care manager is not a medical doctor, he also provides me with a responsible nurse who visits me monthly and asks questions about my general health state. If urgent treatment for care is needed, she will contact a medical doctor immediately.
He's a very fun guy to talk with and we call him "The Colonel".
