The areas chosen for graft transplantation are typically the frontal hairline, the front of the scalp, and the crown,where hair thinning is noticeable. To accommodate the transplanted hair, spaces are created one by one.

The two primary methods for this are the hole slit and the line slit. "Hole" refers to an opening, "slit" to a cut, and "line" to a line. The hole slit technique involves making openings with a needle-like instrument (similar to a syringe) to accommodate the grafts, while the line slit involves making shallow incisions with a scalpel-like instrument for the same purpose. The length of each space is adjusted based on the size of the grafts harvested from the donor area. The choice between the two methods depends on the clinic, but in our clinic, we opt for the line slit technique. Internationally, there seems to be a consensus that the results with the line slit technique are neater, and it imposes less burden on the patient.

One drawback of the hole slit technique is that the skin where the holes are made may be damaged, leading to poor results and increased discomfort for the patient. With the line slit technique, there is no skin damage, and the incisions made with the scalpel naturally close over time. However, it requires more skill than the hole slit technique since it involves delicately creating spaces by gently widening the incisions with a scalpel while implanting the grafts. With skilled staff who possess this technique, I believe that the line slit technique becomes the first choice for autologous hair transplantation.

In our clinic, we have adopted an advanced version of the line slit technique called "micro line slit." This method involves creating extremely small line slits at 0.1mm increments, considering the existing hair or the hair that would have been there. Smaller slits minimize damage to the scalp and result in higher graft retention rates due to the scalp's excellent condition. Furthermore, as the slits become minimal, the downtime due to postoperative swelling and edema is also minimized.
