34日目、出勤1日目
4か月ぶりに患者さんの評価をしました。配属先の理学療法士の診察の合間にちょっと見せてもらう程度ですが。毎週月曜は外来診療日です。業務内容は理学療法というよりも、リハビリテーション科か整形外科の医師の診察に似ています。
理学療法士は病歴を聞き取り、X線や手術が必要であれば依頼箋を書きます。診断名は、脊髄炎、脳性麻痺、先天性奇形、先天性内反足、エルブ麻痺（出産時の腕神経叢損傷）、大腿切断など、想像以上に子どもが多かったです。大人は変形性膝関節症、変形性股関節症、脊椎症、扁平足、坐骨神経痛など整形外科疾患がほとんどです。
座位保持のための椅子や装具が必要な人は、隣の部屋で義肢装具士に相談します。今日は8時から17時の間に49人も外来患者さんを診察したそうです。心配なのは疾患の英語名が分からないこと。これを機に2年間しっかり勉強させてもらわなきゃ。

The first working day
I evaluated patients after interval of for the first time in 4 months. It was just during short breaks between the consultations of the physical therapist at my workplace. Every Monday is the consultation day for outpatients. More so than physiotherapy, the type of work resembles that of a medical examination in a rehabilitation department or orthopedic department.
The physical therapist finds out a patient's medical history, and if necessary, writes a letter to other hospitals requesting X-ray inspection or surgery. Diagnosis are osteomyelitis, cerebral palsy, congenital deformity, club foot, Erb's palsy and above knee amputation. There are many patients of children that are beyond my imagination. On the other hand, adult's diagnoses are gonarthrosis, coxarthrosis, arthrogryposis, flat foot and sciatica. Most of all are orthopedic problems.
If the people need a device to help maintain a sitting position or orthosis, they enter the next room and talk with a prosthetist. Today, between 8 AM and 5 PM, she consulted 49 patients. My fear is not knowing the English names of diseases. I will take this opportunity to learn English for 2 years!
35日目、入所中の子どもたち
今は5歳から18歳程度の26人の子どもが共同生活を送っています。先天性内反足を装具で対応していた子どもが7-8歳頃になるとアキレス腱延長術を行っているようです。重度な内反足に対して下腿切断をした子もいました。術後2-3か月で装具作成と理学療法のために入所している子を見させてもらいました。
傷周辺の筋が硬くなっていて、他動的にストレッチされるととても痛がっています。顔をしかめて痛みに耐えている子どもたち・・何とかしてあげたいな

Children in this center
Nowadays, 26 children who are between the ages of 5 and 18 live together here at this centre. Someone who had club foot was taken care with orthotic, they have surgery that extends their tendon Achilles when they became around 7 or 8 years old. One of them had a severe club foot amputated below the knee. I treated one of the children who lives there to make an orthosis and have physical therapy after surgery which was 2 or 3 month ago.
A muscle around sore is tight, so he feels pain when the muscle is stretched with passive movement. They put up with intense pain and their face is distorted. If there is any way I could do something for them, I would.
36日目、入所中の大人たち
現在は男性10人女性5人が義肢装具の作成と歩行再獲得を目的に入所しています。おおよそ8から12週間入所するそうです。両側切断の人はそれより長くかかるようです。
バイク事故で右大腿切断となった50代の男性は切断したのが1年前、5か月後に退院しました。今は義足を作成して1ヶ月経ったところだそうです。杖なしで歩いています。
日本の義足との一番の違いはソケットです。吸着式ではなく、みんな差し込み式ソケットとベルトを使っています。「断端が抜けやすくないですか？」と聞くと少しゆるくてもちょうどいいとのこと。大腿切断義足は骨盤ベルトが必須。
理由を探ってみたいと思います

Adults in this center
Today, 10 men’s and 5 women are in this accommodation to able to walk again, to make own prosthesis and orthosis. They usually stay between 8 weeks and 12 weeks. Bilateral amputees take long time than one leg amputees.
The man of 50 years or more had right above knee amputation one year ago, left hospital 5 months later from admission. Now, one month since he had made his artificial leg. He can walk without crutches.
A great deference of artificial legs between Japan and Ghana is a socket. They use plug in sockets instead of suction sockets and belts. I asked "Is your stump easy to come out?” he respond “No problem if it is a little loose. “A pelvic belt is necessary for above knee amputees.
I want to find out the reason why they use plug in sockets.
37日目、輸入品の牛乳
ガーナでは牛乳は高級品です。1リットル7セディ。日本円にして約350円です。パックに書いてある表示によるとフランスからの輸入品でした。ガーナでミルクが欲しいというと缶詰めの練乳のようなものが出てきます。「Ideal」というネスレ社製の商品で410gで3セディ、日本円で約150円。
常用のミルクは豆乳にしようかな・・

Import milk
In Ghana, milk is quality goods. It cost 7 Ghana cedi for 1 liter, 350yen in Japan. The milk is import in France according to the label of package. In Ghana, when I said I want milk, they bring me a can resemble condensed milk. It is named "Ideal “made in Nettle company and it cost 3 Cedi for 410g, 150 Japanese Yen
I think soya milk may be in common use.
38日目、PT外来
成人の患者さんもPTを受けるために通ってきています。今日は変形性膝関節症の女性、大腿骨折術後の男性を見させてもらいました。
ここでは通常20分程度で、マッサージ、関節モビライゼーション、筋力強化、歩行練習をしているとのことでした。これまで日本で働いていた時と同じような内容と時間配分で行われていることに親近感を覚えました。
ガーナの人は基本的に皮膚が固い感じがしました。女性は日本だったら人工関節置換術を勧められていそうな状態でした。男性は2か月前に金属プレートで固定する手術を受けたようです。でも、X線写真はありません。今日は関節周囲筋をリラクセーション、そして自動運動をアドバイスしました

Physical therapy for out patients
Adult patients come to have physical therapy. Today, I allowed me to have a look at a woman with gonarthrosis and a man who had a surgery of femur fracture.
Usually, it takes 20 minutes for massage, joint mobilization, muscle strengthen exercise and gate exercise. I found in something congenial to myself, because contents and time allotment have many points of similarity to my work in Japan.
I felt Ghanaian's skin and it was a little hard. The woman's knee condition will be recommended surgery of Total Knee Arthroplasty.
The man had surgery to strap a broken leg with metal plate two months ago. But, he doesn’t have X-ray. Today, I did relaxation of hip joint and advised to him on active exercise.
