イースタン州の隊員がそのうちの一人の活動先に集合しました。ビーズで有名なクロボという町です。廃材のガラスから、色々な種類のビーズができあがります。人の良さそうなおじさんが、一つ一つ手作業で作っています。
できたてのビーズを使って自作のブレスレットと、携帯ストラップが完成！

Leisure activities in Ghana
Volunteers in eastern region gathered at the workplace in which one of them works. The town of name is Crobo, this town is famous for beads. Various kinds of beads are made from glass that is waste lumber. A good-natured man makes one by one using the hands.
A bracelet and a cellphone strap using just made beads of my own making ware finished!
69日目、理学療法トーク
久々に理学療法に関する話ができました。日本語では、こんなにも自分の考えを詳しく伝えられるのかと言葉の違いを実感しました。他の地域の障害者の置かれている状況を聞くと、地域差を感じます。自分ももう少ししたらうちの配属先にアクセスできていない人を探したいと思います。

Physical therapy talk
After a long time, we could talk about physical therapy. I actually felt that difference of language and how I can tell my thinking in Japanese. When I heard the circumstances of people with disabilities in other area, I feel regional differences.
I want find out people they cannot access my workplace a little later.
70日目、足のお値段
兼ねてから気になっていた義足や装具の費用を聞いてみました。
日本ではパーツによって値段が変わりますが、ガーナではどのパーツを使ったとしても統一価格です。成人の大腿義足は900ガーナセディ（約45000円）、下腿義足は700ガーナセディ（約35000円）整形靴は45ガーナセディ（約2250円）12歳未満の大腿義足は500GCD（25000円）で、子ども用は若干低価格です。
健康保険は使えず全額自費です。でも払えない人には配属先が金銭的な援助をしています！貧しくてお金を持っていない人がいるから、どんな高いパーツを使おうと一律価格なのだそうです。カトリックのNGOだからということもありますが、持てる人が持てない人を助けるのが当然という意識が根底にある国なのです。
シンプルで分かりやすいと言えばそうですが、バックに寄付してくれる人がいるからできていること。ビジネスにしようとしたらこうはいかないのだろうなぁ。

The price of leg
I asked about price of prostheses and orthoses that I have concerned since long time.
Price depends on parts in Japan, but in Ghana, it is same price if it uses any parts. Above knee prosthesis for adults cost 900 Ghana cedi (about 45,000Yen), below knee prosthesis costs 700 GHC (35,000Yen), orthopedic shoes cost 45 GHC (2,250Yen), above knee prosthesis costs 500 GHC (25,000Yen), prosthesis for children are low cost.
They pay themselves all for them, because national health insurance cannot support. But, if patients couldn't pay, my recipient agency gives them financial aid! The poor can't pay for prostheses and orthoses, that’s why prostheses and orthoses have a uniform price whatever they use any parts. I don't suppose that's the reason that my workplace is Catholic NGO, national sentiment in Ghana that "the haves give to the have-nots" underlies this price-setting.
It is simple and easy to understand, it can maintain as there are donors behind my workplace. I think this way the business may not pay.

頚椎骨折術後の外来患者さんがCT写真を持ってきてくれました。X線とCT以外にMRIが入っていたことに驚きましたが、さらに3D－CTの写真まであったことにびっくり！
ガーナで3D-CTにお目にかかれるとは思いもよりませんでした。途上国だと思って甘く見てはいけません。
あるところには何でもある、でも、ないところには何もない。この格差を少しでも少なくすることが必要なんだろうなと思います。

3D-CT
An outpatient who was postoperative of cervical fracture brought an X-ray. I was surprised there is a MRI except an X-ray and a CT, furthermore, she had a 3D-CT.
I had no idea I could be shown 3D-CT in Ghana. Don't underestimate developing countries.
Some place has everything, but other place has nothing. I feel need to abolish such a gap.
72日目、身体測定
大人の切断者達の毎週水曜日の午後の日課は、ディスカッションです。これまでは、火傷、出血などについて話あったようです。なんと2時間連続で話し合っていました。
先週「何かレクチャーできることはないか」と言われました。とても2時間話すことはできないと思い、彼らの身長・体重を測ることにしました。計測前に肥満や代謝についてのお話しを少し。
講義は受けたものの、なかなか振り返ることのなかった資料が役に立って良かったです。

Measurement of height and weight
A daily task for adult amputees on Wednesday afternoon is discussion. They talked about "a burn”, “bleeding" and so on in the past. Their talk lasted for two hours.
The last week, I was asked "Do you have something lecture what you can do?” I thought that it is hard to talk for two hours, so I decide to measure their height and weight. I talked a little about obesity and metabolism before measurement.
I'm pleased that some handouts of lectures that I couldn’t go over came in very useful.
73日目、関節拘縮症術後の水中運動療法
関節拘縮症でハムストリングスや内転筋、アキレス腱周囲、足部など8か所も手術痕がある7歳の男の子。5㎝の脚長差もあります。下腿は直径3㎝ほどの太さしかありません。
プールに入ると足が軽くて浮いてしまい、自力で立位が取れません。ふだん、両松葉杖で前傾位、膝はロックしたままの状態で歩行しているので、二足歩行の姿勢でどうやって足を動かしたらいいのか分からないみたいです。
次回、重りを使って足の動かし方が分かるかためしてみるかな。

Aqua exercise for child who have postoperative arthrogryposis.
A boy who is seven years old has 8 scars on hamstrings, adductor, Achilles tendon and foot. He has a gap of 5cm between his both legs. His leg is only 3cm thick.
He can't stand himself because his leg is too light. Usually, he walks with his body bend forward, his knees were locked, using crutches, so he doesn't know how to move his legs in a posture of bipedalism.
Next time, I'll try to use a weight if he knows how to move his legs or not.
74日目、短肢の装具
ここには、各地から装具を作成したい患者さんが集まるので様々な症例に出会います。今日評価したのは先天性の下肢形成不全の3歳の女の子。
大腿骨は反対側の半分の長さしかありません。下腿と足部の位置関係もずれています。でも、その足で立つことができます。長下肢装具に似たキャリパーという装具を使っています。中間部分に足の裏を載せる部分を付けて、反対側の足の長さに合わせています。
トラブルなく使えていくのでしょうか？また来週見せてもらおう。

An orthosis for micromelia
I met various case patients who need orthosis from everywhere. I evaluated a three-year-old girl who has congenital aplasia.
Femur is only half the length of that of light side. It is out of alignment with lo dower limb and foot. But she can stand on the leg. She uses an orthosis called caliper, it looks like Knee Ankle foot orthosis. There is a part for putting the foot on it, and then make same height of another leg.
I don't know they use it without trouble. I'd like to see her next week.
75日目、山道を行く
乗り合いバス「トロトロ」で少し離れた町に行きました。これまで「トロトロ」で通ったことのある道は、おおむね舗装されていたのですが、今回通った道は舗装されていない道。遊園地のアトラクションのように左右に上下に揺れます。
途中、井戸水をくみ上げる手動ポンプと、それを囲むようにして立ち並ぶ土壁の家を横目に見ながら、1時間弱揺られると、下に広がる緑を一望できる高台に来ていました。
パンクもせず、横転もせず無事に帰ってこられて、一安心。

Going in a mountain trail
I went a town at a short distance by bus called "trotro". The road that I passed before by “trotro” is paved, but the road that I passed today is not paved. It joggled up and down, left and right as it like an attraction at the amusement park.
I came back alive, no blowout and no rolling over, I breathed easier.

Ghana's most beautiful part 2
Today, representatives competed in fashion sense. The designers are in chage of them one by one walked on a stage.
This time, the representative of Volta region won. It broadcast live until 11PM!
77日目、待ち人現れず
テレビ局が取材に来るから、プールエクササイズができる準備をしておくように、と言われました。
しかし、1時間待っても現れず。子どもたちは昼食の時間に入ったため午後まで待機。結局何の呼び出しもなく水着を着たまま1日が終了。
もう待機している必要はないと、教えてくれたらよかったのに。

No expected visitor came
I was said to prepare for aqua exercise, because a TV crew would come. But they didn't come even I waited for one hours. I waited for an opportunity, but closing time came while wearing swimwear.
I would like to be known I didn't need to wait.
78日目、靴の修理
靴の修理と言っても、患者さんのではなく私の靴。地元のマーケットで、15 GHCで買ったサンダルが1ヶ月半で真っ二つに割れました。ラッキーなことに私の配属先は義肢装具センター、というわけでソールを交換してもらいました。
今度は何日持つかな？

Repair of shoes
repairing of shoes, but not for patients but for me. My sandals which I bought for 15 Ghana cedi in a market were broken in two while one month and half. Fortunately, my work place is Orthopedic training center, therefore soles were changed for me.
How many days will it last?
79日目、BMI
水曜日は切断患者さんのディスカッションの日。先週測った伸長と体重から自分のBMIを計算するという作業をしました。
みなさんほとんど25以下。足がない分体重が軽くなって低く出るのかも！？

Body mass index
Every Wednesday is a discussion day. We calculated their BMI from own height and weight that measured last week.
Almost all are under 25. It may be low because they have short leg!?
80日目、マーケット渋滞
私の住む町では月曜と木曜がマーケットデーです。マーケットデーとは、マーケットに集まるお店の数が多い日を言います。お客さんが集まるので、それに伴って行き交う車も増えます。
そんなわけで、マーケット近くの道では車がなかなか進まない、バスロータリーは大混雑。露店の場所を移動するよう協力してもらって道路の車線を1本増やすとか、できないものかしら？

A traffic jam due to a market
Monday and Thursday is a market day in my town. Market day is a day that a large number of stores open in the market. Following on many customers, the number of cars increases.
That's why cars don't move on the road near the market, bus rotary is crowded. Why don't ask a roadside stand to move the place and make one more lane?
81日目、短肢の女の子、その後
先週みた短肢の子の2回目の評価をしました。大腿も下腿も反対側の半分程度しかありません。配属先のPTによると将来的には下肢切断の手術を受けるのではないかとのこと。
こんなケースにどんな手術が適応なのかな

The continuation of an orthosis for micromelia
This is second time assessment of a girl who has micromelia I assessed last week. The length of femur and tibia is half of the other leg. According to the physical therapist of my workplace, she may be amputated someday.
I wonder what kind of surgery indicated for such a case.
