明けましておめでとうございます。
まだECFMG来てませんが、今日からSTEP3の勉強始めようと思うので、UWの内容をなうにつぶやこうと思います。
でもしばらくは多くても一日一問しか解かないつもりなので、一日一なうかな。

HBVはMembranous glomerulonephritis

HBV, HCV, syphilis, malariaに関連するのがMembranous glomerulonephritis
Heroin, HIV, DM, sickle cellに関連するのがFSGS
学生時代にもやったはずだけど、なかなか覚えるられないね雪

VZV潜伏期間10-21日, うつる確率70-90%

VZV

incubation period 10-21days (gen. 14-17days)
contactで感染する確率　70-90%
感染性があるのはrashが出る48時間前からrashが消える(crusted)まで

Exposure criteria
continuous household contact
playmate for > 1hr indoor
hospital contact
Mother with onset of chicken pox <5days before or <2days delivery

High risk person
(IC children, healthy adolescent(15>yrs) and adult, pregnant women,
Newborn of mother with onset of above mentioned, hospitalized preme infant)

VZIG is indicated in high-risk persons exposed to VZV.
within 96hrs of exposure (preferably within 72hrs)

It may not prevent infection but reduce the disease severity.

Post-exposure prophylaxis は96hr以内のVZIGかacyclovir.

Raloxifene- DVTに注意

Raloxifeneはbreast, vaginaともにantagonistなのでCa.のリスクはないが、thromboembolismを↑するので、DVT既往のPtには禁忌。

Tamoxifeneはendometrial Ca.のリスクあり。

Biophysical profile (BPP) scoringとマネージメント

NST (reactiva)
Fetal tone (手足の屈伸）
Fetal Movenents (at least 2 in 30min)
Fetal breathing movement (at least 20sec in 30min)
Amniotic fluid volume(single pocket greater than 2cm in vertical axis)

この5つの項目それぞれに0点(-)か2点(+)つける。

合計8-10点で、正常
羊水減少ありで8点の場合はdeliveryを考慮
羊水減少なしで6点の場合は37週以降でdeliveryを考慮
37週未満の場合は、24時間以内にBPPを再チェック⇒スコアに改善なければdeliveryすべき
羊水過少ありで6点の場合は32週以降でdelivery, 32週未満で毎日モニタリング
4点未満で26週以降の場合はdeliveryの適応

PCOSの高血糖にはMetformin

glu tolerance testでgluの2時間値が140以上でinsulin resistance, 200以上でDM

PCOSでMetformineを使うbenefitは４つ
　1. prevent type2 DM
2. helps losing Wt.
3. clomiphenとの併用で、排卵を誘発する
　4. androgen productionを抑えることで、hirsutismにある程度効果的

residency programが要求しているアメリカでのclinical experienceを満たしつつ、アメリカ人によるLORがgetできるような、FMG向けのexternshipが、色々用意されていてびっくりした。

聞いた事もないような病院や医科大学で、HPにビジネス臭がただよい、胡散臭い！

ハワイ大学でのエクスターンシップもお金を要求するようになったって話ですが、matchingにおいても
外国人はビジネスの対象にされているんですねー。悲しい･･･
