今日は手術後の生検の結果が出たのでそのご報告です。
2025年2月に肺癌S4と診断を受けて、治療をしなければ余命約半年と言われた私ですが、8月にサルベージ手術を受けました。
その際に取り除いた癌の生検結果が先週でまして、癌の92%が消失（死滅）という結果でした。8%はまだ癌が残っていたのですが、その癌を含む目に見える癌はすべて切除できたという結果です。
EGFRという遺伝子変異陽性癌で、タグリッソ（オシメルチニブ）という分子標的薬を毎日1錠服用しています。 分子標的薬はよく効くということですが、具体的にどれほど良い結果だったのか主治医からは詳細な情報がいただけなかったので、タグリッソを使用した場合の効果についてChatGBTに効いてみました。↓
Chat GBT： 30%以上縮小すれば「奏効」とみなされる中で、90%超は非常に稀で素晴らしい反応です。通常、化学療法や分子標的薬を使ってもここまで腫瘍細胞が消えることは多くありません。特に肺癌では、MPRや完全奏効を達成した患者さんは予後が良い傾向が世界的に報告されています。医学的に見ても「極めて良好な治療効果」といえます。
信じるか信じないかは君／私次第（笑）ですが、半年で92%消失したことは事実ですし、その結果サルベージ手術を受けられて今がありますので素晴らしい結果だと思っています。
あのまま手術をせずに薬だけを飲んでいた場合ですが、薬の効果持続期間は中央値で18ヶ月前後とのこと、個人差はあるとのことですが遅かれ早かれ耐性ができ薬が効かなくなるので、その後は別の治療を開始する必要があります。
稀なケースとして手術がうけられた私ですが、今後数年以内の再発率が高いのもわかっています。しかしながら、主治医いわく根治の可能性あるので手術を行ったとおっしゃってくださったので、手術ができて本当に感謝です。そして根治したと信じて進みます。もちろん再発の可能性が高いので引き続き分子標的薬の服用はつづいています。
癌になったのが夫や子供ではなく私で本当に良かった。

I'm writing to update you on the results of my post-surgery biopsy.
I was diagnosed with lung cancer (S4) in February 2025 and told I had about six months to live without treatment. In August, I had salvage surgery. The biopsy results from the removed tumor came back last week, and they showed that 92% of the cancer was gone (necrotic). While 8% of the cancer remained, the results showed that all visible cancer, including the remaining cells, was successfully removed.
My cancer has a positive EGFR gene mutation, and I've been taking one tablet of the molecular targeted drug Tagrisso (osimertinib) daily. I know these targeted therapies are effective, but my doctor didn't provide specific details on how good this outcome was, so I asked ChatGPT about the effects of Tagrisso.
Here's what ChatGPT said: "In cases where a reduction of 30% or more is considered a 'response,' a reduction exceeding 90% is an extremely rare and excellent reaction. It is not common for tumor cells to disappear to this extent, even with chemotherapy or molecular targeted drugs. Whether you believe that or not is up to you (laughs), but the fact is that 92% of the cancer disappeared in six months. Thanks to this outcome, I was able to have the salvage surgery, and I'm here today, so I consider it an amazing result. If I had chosen to only take the medication without surgery, the median duration of its effect is about 18 months. Although it varies from person to person, I would eventually develop a resistance, and the drug would stop working, requiring me to start a different treatment.
As for me, I was able to have the surgery in a rare case. I know that the recurrence rate is high in the next few years. However, my doctor said he performed the surgery because there was a possibility of a complete cure, and I am truly grateful for that. I will move forward believing that I have been completely cured. Of course, since the possibility of recurrence is high, I will continue to take the molecular targeted drug.
I'm so glad it was me and not my husband or children who got cancer.
