こんにちは。今日はリプロダクション 東京の初診について書きたいと思います。不妊治療の病院はふたつめだから、検査内容も先生と話すことも慣れてるよと思いつつも、初めていく病院は緊張。
初診は平日11:00amの予約。
初診の場合は予約30分前に来て欲しいとのことで、10:20到着。登録票に記入し、現在通院しているび病院からの紹介状提出。病院の受付や予約方法のガイダンスを受ける。
昨日他の病院で移植したばかりのことを伝え、先生との面談から開始することに。
クリニックの一番奥のホールが待合室になっており、そこで呼ばれるまで待つことに。60人くらいが入る待合室は照明があまりなく、薄暗い感じ。平日なのに８割くらいが埋まっている。
自分の順番が来ると、正面にあるスクリーンに自分の番号と行く部屋が表示される仕組み。
Free-wifiあり。
専用のアプリを携帯に入れておけば、待合室にいなくても呼び出しに気づけるようになっている。
待合室に移動したのか10:40くらい。
12:00過ぎても呼ばれず、受付に確認しにいくと、「次の順番です」と言われる。
待合室に戻り20分位経ったところで、目の前のスクリーンに自分の番号が表示される。
診察室で先生と面談開始。
事前に送っていた、他の病院で行った検査結果が電子カルテにインプットされて写されている。その内容を見つつ、今までの記録を口頭確認。
着床不全検査と転院を考えている旨を先生に伝える。
先生曰く、昨日の判定が陰性だったら不育症検査をやればいいとのこと。
正直、陰性な気がしていたし、採血回数を少なくしたいから、必要なものを一気にやりたいとお願いしたら、不育症検査までやってくれることに。
着床してないから不育症検査はいらないと思っていたけれども、今の医療技術ではその境目をはっきり判別できないらしい。
受精卵の発育不良や着床をしない場合を「真の不妊症」といい、着床以降で妊娠反応ない、化学流産や流産する場合を「真の不育症」というとのこと。
これまで陽性反応が出たことなくても、それが着床したのかどうかは判定する手段がないとのこと。
なので、まずは着床不全検査と不育症検査のうち、今まで実施してこなかった検査をやることに。
追加検査内容
子宮内膜炎検査（まえの病院では陽性判定され、抗生剤を処方されたのみ。炎症がなくなったことを確認するため）→生理後7-10日目に実施
子宮収縮検査→本日実施
不育症検査→本日実施
亜鉛銅検査→本日実施
再び、待合室で検査が始まるまで待つ。10分位したら再びスクリーンで呼び出し。採血室へ移動。
採血は１０本。採血しにくいらしく、看護師さんがトントンと血管の周辺を叩く。不妊治療を初めて最大の採血量。
採血部屋には、採血した血液を分析する機械がガンガン動いていて、それを眺めつつ採血に耐える。
結果は２週間後に出るとのこと。
再び待合室へ移動。
内診室に呼ばれ、子宮収集検査。
内診室は噂通り、明るく超オープンスペース。脱衣場所は個室型。病院によって違うけれども、リプロ東京では、スカートを着ている場合は下着のみ脱げばいいとのこと。脱衣室から、内診室にドアがあり、ドアを開けたら明るい部屋が。普通に医療スタッフが行き来してる中に椅子が置いてある。待合室より明るい！そこに座り、先生がきたら椅子が上がる。上がると前にカーテンを引いてくれ先生と目が合わずにすむ仕組み。カーテンのおくで色々な人の足音や話し声が聞こえるのはなんとも落ちつかない。。
エコーで子宮内の動きを3分間録画。録画したビデオを、早送りで見て子宮内が動いているかどうか確認するらしい。
動いていると受精卵をはじいてしまい着床しにくいとのこと。
一通り検査が終わったら、ファイルを受付に提出して受付前にある椅子に座りつつ会計待ち。30分くらいで会計が完了。精算金額に関わらずカード決済ができるとこ、保険診療と自費診療がまとめて精算できるのはありがたい！
検査をどんどんやってくれるところや、説明がわかりやすかったのが好印象。前の病院は、お会計を間違えたり、こっちから聞かないと教えてくれなかったりすることに不信感が募っていたので、、
転院を決意！
お会計して出たのが2時過ぎ。
今日は会社を１日おやすみにしてよかった。病院は混雑具合によって終わる時間が前後するから、会社の休みの取り方が難しい。

Today, I am going to write about my first visit experience at Reproduction Clinic Tokyo. I was a little bit nervous for this visit because first experience always has new things although I knew what will be required and need to talk with doctors from previous clinic.
My appointment was 11am on Friday. For the first time visitor, the clinic asked the first time visitor to arrive the reception 30min. before the appointment to fill in the registration form. After the registration, I handed in the referral letter which includes the data of my treatments and blood examinations from the clinic I am going now. Then, I had a guidance how I check-in the reception and make a next appointment.
As I transferred an embryo on the previous day, I requested to see a doctor not a pelvic examination. The waiting room locates on the backward of the clinic for about 60 patients. The seats were about 80% occupied,
In front of the waiting room, there is a big screen to show the client number and the room number to go. Free-wifi is equipped. Patients don’t need to wait in this room if you have the special app on their phones which let you know your turn. I started waiting my turn around 10:40am. My turn didn't come by 12:00am and to confirm my turn, the receptionist told me that my turn is the next.
Then I went back to the waiting room and they called me in 20min.
I met a doctor at a consultation room, and we looked through the examination result from the previous clinic.
As I couldn't get pregnant during the treatments in the previous clinic, I asked the doctor to conduct implantation failure inspection. The doctor told me that inspection I had was not enough and decided to have additional inspections.
Then we decided to have both additional implantation failure inspection and recurrent fetal loss inspections.
The additional tests were below.
-Chronic Endometritis Inspection
-Uterine Contractions Inspection
-Zinc Copper Inspection
-Recurrent Fetal Loss inspection
Chronic Endometritis Inspection will be conducted 7-10 days after the day next period starts.
After the discussion I went back to the waiting room and waited for about 20min.
Had my blood collected for 10 bottlesat blood collection chamber. The result will be turned out in two weeks.
Next inspections was Uterine Contractions Inspection.
Recorded ultrasound video for 3min. Then doctor check the video 10times faster and confirm if endometrial is moving or not. Movements of endometrial interfere the implantation of eggs. The result will be turned out in two weeks approximately.
After all today's inspections, put the file at reception and they calculate the all costs.
The good points of this clinic were below.
-Calculate the cost for both insurance coverage and non-insurance coverage at the same time, which will reduce total costs.
-Can pay all costs without minimum amounts by credit card.
Today, I took a whole day off from my work because I didn't know when I would finish all examinations. At my current company, I don't have any flexible time off system. I can only choose a half day off or a whole day off, which makes really hard for working people to balance work and fertility treatments.
