21.自毛植毛について 『PRP（多血小板血漿）を使用する医学的根拠』
自毛植毛におけるPRP（多血小板血漿）の有用性について
※エビデンス（論文を提示）に基づいて説明していきます
PRPとはplatelet-rich plasmaの略で、日本語では多血小板血漿（たけっしょうばんけっしょう）と呼ばれ、血小板を濃縮したものです。
血小板の成長因子による組織修復能力を利用した再生医療です。
PRPに含まれる成長因子の特徴
PDGF（血小板由来成長因子）：細胞増殖を刺激、血管新生、コラーゲン産生
TGF-β（形質転換成長因子）：細胞外マトリックス形成、骨代謝調整
VEGF（血管内皮成長因子）：血管内皮細胞形成
FGF（線維芽細胞増殖因子）：内皮細胞・線維芽細胞増殖、血管形成
EGF（上皮細胞成長因子）：上皮細胞成長促進、血管新生、創傷治癒促進
こうした組織修復能力を期待して、PRPは臨床の現場でも難治性皮膚潰瘍など皮膚科領域、変形性膝関節症など整形外科領域、口腔外科領域などで使用されています。
男性型脱毛症においても発毛促進効果を期待する治療が試みられています。１）〜５）
有効とする研究結果が多くみられますが、PRP療法を脱毛症の標準治療とすることは現状においてはさらなる検討が必要という位置付けです。
植毛に関連して、植毛時にPRPを併用することで生着率が向上することが報告されています。6)7)8)
創傷治癒促進効果がPRPによる難治性皮膚潰瘍治療において認められています。9)10)11)
安全性においてPRP療法は自分の細胞を使用するので安全性が高く、副作用が少ないとされています。12)
結論として安全性、発毛促進効果、生着率向上、創傷治癒促進効果の点から植毛手術におけるPRPの使用は有用と考えられます。
