ELLDOPA試験の
結果を拠り所に
日々の治療で
安心して投与されている
レボドパ。
パーキンソン病の
ドパミン補充療法は
有名ですが、
ドパミンは血液脳関門を
通過することが出来ません。
ドパミン前駆物質として
レボドパが使用されるワケです。
レボドパは
血液脳関門を通過し
脳内の脱炭酸酵素によって
ドパミンとなる、
いわゆる、
プロドラック
として作用します。
困ったことには、
脱炭酸酵素は脳内だけでなく、
末梢にも存在し、
投与されたレボドパのほとんどが
ドパミンへと変換されてしまいます。
レボドパの脳内移行率を
上げるために、
末梢性脱炭酸酵素阻害剤があり、
これにより、
レボドパの投与量を
減らすことができ、
副作用も減らすことができます。
末梢性脱炭酸酵素阻害剤には
カルビドパやベンセラジドがあり、
血液脳関門を通過できません。
