141日目、現地語でも話せるようになりたい
挨拶は現地語でできるようになってきたけれど、必要な情報のやり取りはまだ難しいです。
今日は母子入所している子どもを担当する日。「立って」「座って」「寝て」「ここは痛い？」は必須の用語。お母さんに「筋肉が固い、柔らかい」の現地語を教えてもらいました。
Nov 20th (141 days) I want to talk in a local language
I started greeting in a local language, but it is difficult to talk about necessary information.
Today, I was in charge of taking care of children who were admitted with their mothers. "Stand” “sit down” “lay down” “Do you feel pain here?" are indispensable words. I was learned from mothers some local languages that mean "muscles are stiff, muscles are soft".
142日目、不全麻痺の脊髄損傷
ガーナに来て初めて脊髄損傷の患者さんを対応しました。4年前に鉄塔から落ちた20代の男性。
ガーナ式長下肢装具と歩行器で屋外も歩いているのだそう。エルボークラッチで歩きたいとやってきました。1人暮らしで仕事もしているとのこと。生活の様子を見に行ってみたいな。
Nov 21th (142 days) incomplete spinal cord injury patient
This is the first time that I have ever treated a Ghanaian spinal cord injury patient. He is twenty years or more who had fallen down from a steel tower four years ago.
I heard that he can walk outside with Ghanaian-style long leg braces and a walker. He came here that he wants to walk with elbow crutches. Furthermore he lives alone and works. I want to see his daily life.

配属先の職員がファームに卵を買いに行くので一緒に買うかと聞いてきました。いくらか聞くと「30個で8セディ」だと言われました。日本円で約400円です。
マーケットで買うより1セディ安いだけですが、ビニール袋入りの卵を割れないように気を付けて買って帰るのに比べたら断然良いです。今度場所を教えてもらおうっと。
Nov 22nd (143 days) Three dozen of eggs
One of staffs asked me that if I want to buy eggs or not, because she would be going to a farm. I asked her the prices, she said "30egs for 8 Cedis". It cost 400 Yen.
Those are cheaper 1 Cedi than that I bought in a market, but it is prefer to bring eggs in a plastic bag carefully and prevent to be broken. I let me know where the farm is.

活動をはじめて3か月ちょっと経過しました。月曜は毎回40人前後の患者さんの診察補助、火曜は外来患者さん5-6人の対応と切断者の適合評価ミーティング、水曜は切断者対象のワークショップのファシリテート、木曜は集団体操のリードと3-4人の個別訓練、午後は5-6人の子どもとプールエクササイズ、金曜は母子入所している子ども10人前後の理学療法。あらためて振り返ってみるとフル活動だったなぁ。
今までは英語と現地語に慣れ、業務内容を覚えて課せられた業務をやりきるだけで精いっぱいだったけど、これからはちょっとずつ自分のアイデアを提案していきたいと思います。
Nov 23rd (144 days) Quarterly report
It took 3 months and a few days from started work. I support to consult about 40 patients every Monday, I took care 5-6 outpatients and have a meeting about assessment of fitting for amputees every Tuesday, facilitate of workshop for amputees on Wednesday, lead group exercise and instruct 5-6 children individually and provide hydrotherapy every Thursday, do physical therapy for around 10 children who were admitted with their mother on Friday.
It was hard work that I was familiar with English and local language, remember routine work and finished the duty work. But I will talk about my idea little by little from now.

9月の初めから毎週日曜日にやっていたテレビ番組、「ガーナ's モーストビューティフル」。
必要なのは見た目だけではありません。3か月弱、ガーナにある10の州の代表者が、毎回いろいろなパフォーマンスをして競うというもの。
ファッション、ダンス、スピーチ、演技、社会問題を問う寸劇などなど、今日がやっとファイナルでした。優勝者はアッパーイースト州代表、8000ガーナセディの賞金と車、服、その他色々もらえるそうです。学校の先生の1ヶ月の給料が4千円というところもあるらしいので、その人の給料の約8年分になります。みなさん気合が半端なかったのも分かる！
Nov 24th (145 days) the final round of MS Ghana 2013
They watch one of the TV programs every Sunday that started from the beginning of September, that name is "Ghana's most beautiful"
Candidates need not only looks. Ten representatives from each region compete others several performances every time for 3 months.
Topics are fashion, dance, speech, action, a short play about a social problem and so on. Today's performance was the final. The winner is a representative of Upper East region, she got 8,000 Ghana cedi, a car, clothes and more. The salary of primary school teacher is 4,000 Yen for 1 month, so that 8,000 Ghana cedi is equal to their salary for 8 years. I agree that they were fired up enough!

同僚の一人が体調不良で休んでいるため、子どもたちの集団体操を終え、他の同僚と一緒に診察業務を回すことになりました。
9時間で46人、朝8時に会った子と16時にやっと診察室で再会。時間を指定してもそのとおりに来ないから先着順に整理券を配って待ってもらっています。患者さんもほんとうにお疲れさま。
Nov 25th (146 days) waiting for 8 hours
One of the colleagues absent from work because she got out of condition, I finished instruct a group exercise for children, became to do consultation with another colleague.
We consulted 46 patients in 9 hours, I met again a girl at 4 p.m. whom I met her at 8 a.m. We distribute numbered tickets in order of arrival and let them wait because patients don't come on time if we had an appointment. Thanks for all patients.

たまご30個に続いて、パパイヤ20個を手に入れました。もちろんパパイヤおじさんから。おじさんの家の近くを歩いていると「どこに行くんだい？」「最近来ないじゃないか」と呼び止められました。
「熟れたのも欲しいかい？」と言いながら長い木の枝で次々と手慣れた手つきでパパイヤを収穫してくれました。今度は何を作って持っていくかな？
Nov 26th (147 days) Massive items series Part 2
I got 20 papayas following to 30 eggs. It is a matter of course that from Mr. Papaya.
I walked beside of his house, he called me "Where are you going?" "You haven't came in these days"
He said to me "Do you want ripe one?" and gathered papayas one after another with skilled hand and a long branch.

今日は母子入所部門の担当日、だったはずが、全部門の人を少しずつ対応することに・・
朝一からダイレクターに脊椎症の外来患者さんの運動指導を頼まれる、スタッフの一人が病院に行ったとの連絡が入る、母子入所の一人を対応、子どもの集団歩行を担当する人がいないため代わりに一緒に歩く、外来の人が来ているから見て欲しいと頼まれ対応、本来の母子入所の子どもの対応、入所するために来たという人を同僚に連絡、成人切断者の1人が装具が合わないと訴え対応。
日本だったらクレームの嵐だろうな。突然スタッフがいなくなるのはどうにかして欲しいです。
Nov 27th (148 days) In charge of whole department
Today I would in charge of children with mothers department, but I had to take care of whole department.
I was asked to instruct exercise for outpatient who had spondylosis, we got information that one of the colleague went to a hospital, I took care one patient, I went group waking with children because of no staff to walk with them, I was asked to treat one outpatient again, I took care of children that I was in charge of originally, I called a colleague to see a patients to come for admission, I adjusted an adult patient's orthosis.
I suppose that it might be storm of complaints if it would be in Japan. I let them deal with when staffs suddenly take off.
149日目、見える化ボードリニューアル
それぞれの子どもに理学療法スタッフの担当を決めていないので、誰が個別の理学療法が済んで、誰が済んでいないのか分からない状態でした。
まだ個別練習していない子どもが分かるようにする掲示板を先月模造紙で試作しました。今日は使われていなかったホワイトボードを再利用してリニューアル。有効活用されることを願います。
Nov 28th (149 days) Renewal of a visualized board
We are not in charge of take care for each child, so that we are not able to grasp the situation that who had finished individual treatment and who had not yet.
I made an experimental notice board that to realize children who had not been done individual training with a vellum paper on last month.
Today I renewed it with an old white board that had not been used long time. I hope it will be used effectively.

いつものようにホストマザー宅の横を通りかかると、たくさんの女性たちが料理をしていました。見てみるとバンクーを鍋いっぱいに作っています。あつあつのバンクーをお椀ですくって小さなビニール袋に入れて口をねじってクーラーボックスへ。50個以上はありそうです。聞くと、明日のお葬式のための準備だそうです。私も、先日亡くなったおじさんにお別れを言いに行こう。
Nov 29th (150 days) Massive items series Part 3
I saw many women cooked a lot of dishes when I pass through beside of my host mother's house.
They made Banku full of a pot. They take with a bowl and put hot Banku in a small plastic bag, then twist the end of the bag, after that they stocked it in a cooler box. There may be more than 50 bags. They prepared for tomorrow’s funeral. I will go to say good-bye to the man.

朝から音楽が聞こえてきました。おじさんのお葬式、高校の先生だった彼の式は学校で行われました。ガーナ国民の約半数がクリスチャンで、彼の式も教会式です。
屋外に赤と黒の屋根のテントを立てて、参列者が座ります。大規模な式で、400～500人はいたように思います。彼は前方中央の棺桶の中で眠っていました。日本と違って、全身がよく見えました。聖歌隊の合唱、生い立ちの説明、お祈り、参列者からの寄付、6人の男手に担がれて墓地へ移動。棺桶ごと墓地に眠るそうです。時々おしゃべりしてくれてありがとうございました。
Nov 30th (151 days) Good bye
I heard music in the morning. It is for a funeral of an older Ghanaian man, held at a school because he was a teacher of senior high school. Ghanaian people of about half are Christian, so that his ceremony is also held by fathers.
They pitched tents colored red and black, and then, attendees sat down under that roof. It was a large ceremony, I suppose that we had 400-500 attendance. He was lying down in a coffin in the center of the space. Unlike Japan, I could see his whole body. Chorus by a choir, speech of his early background, prayers, monetary offering by attendance, movement to a cemetery lifted by 6 men. He is buried with a coffin. Thank you so much for talking with me once in a while.

Nowadays, a lot of JOCV work for people with disabilities in foreign countries.Those countries are Southeast Asia, South Asia, the Middle East, South America, Oceania and Africa. Everyone has own distress and problems are. We have 19 months from now. I'll do my best to my goal!

入所していた人は定期的な診察にやってきます。今日は8月から9月にかけて入所していた5人に会いました。装具の修理が必要な人、順調に回復している子、いろいろいます。次回は誰に会えるか楽しみです。
Dec 2nd (153 days) Meeting again
Patients who were discharged from here come and see at regular intervals. I met 5 patients who were admitted on August to September.Those who need repairing of orthosis, recover smoothly and so on. I'm looking forward to seeing somebody next time.
154日目、外来は大忙し
月曜の診察同様、PT外来も予約制ではないので、誰が何時に来るか分かりません。今日は9時半から片麻痺のおじさん、脊椎症のおばさん、坐骨神経痛のおばさん、頸椎症のおじさんなどなど、気付いたら5-6人が待っていました。
物理療法機器がないので、自主トレを指導して待ってもらいながら3人同時進行。ホットパックが欲しい・・そんな忙しさの中でも嬉しいことがひとつ。不全脊損の患者さんが4年ぶりに初めて松葉杖歩行ができました。バリアだらけの生活を生き抜いているガーナの人たちは腕力が素晴らしい！
Dec 3rd (154 days) Outpatient department were all rushed off my feet
As same as Monday's consulting, physical therapy for outpatients has not the reservation system, so that I don't know who will come and when they will come. I saw a hemiplegia old man, a spondylosis woman, a sciatica woman, cervical spondylosis man and so on. When I became aware of waiting, 5 or 6 people were sitting.
I instructed self-exercise to them and treated three patients at the same time, because we don't have physiayrics equipment. I need hot packs. While I have had so much to do that I had a glad thing. An incomplete spinal cord injury patient was able to walk with crutches for the first time in 4 years. Ghanaian people's power of arm is great, as might expect from survival in surroundings that full of barriers!

来客があるというので行ってみると、大勢の外国人女性がいて、大量の食糧がありました。マットに座った子どもたちが持っているお皿に次々と食べ物が配られました。度々このような経験をして育った子たちは、外国人は何かくれるものだという意識が植えつけられるのかも知れません・・

I went to a veranda to see visitors, many foreign women and a lot of foods were there. They served food to dishes that sitting children on a mat had held. Ghanaian children, who have grown up with that kind of experience, might be planted that foreign people always give them goods...

同僚がクリスマスプレゼントに4人お揃いのワンピースを作ってくれると言ってくれました。ドレスメーキングの仕事をしている同僚の娘さんが採寸しに来てくれました。かわいいのができるといいなぁ!!

西アフリカで最大のマーケット、セントラルマーケットがクマシという町にあります。現地の人でも迷ってしまうというほどお店が密集しています。同僚へのクリスマスプレゼント用に布地を購入。急いで作らなきゃ！

Central market is the biggest market in West Africa that is in Kumasi towｎ. Many stores stand roof by roof there, so that even a local people get lost in the market.I got some materials to make Christmas present for colleagues. I have to make quickly.

カクン国立公園を訪れました。天然の木々の間に吊り橋が7本かかっています。まるで大自然にあるアスレチック公園。幅40cm、地上40ｍに位置しています。無事に生還、楽しかったー、10州あるうちの5州を制覇！そのあと、エルミナ城を訪れ、初めて大西洋の海に足を踏み入れることもできました。
Dec 7th (158 days) first time to Central region
I have been to Karun National Park. There are seven suspension bridges between natural trees. It looks like an athletic park on the vast of nature. It is 40 centimeters wide and 40 meters aboveground. We enjoyed and survived safe. I have been 5 of 10 regions!
After that, I visited Elmina castle. This is my first time setting foot in the Atlantic.

今日はケープコーストへ。130年前はこの町が首都でしたが、今はアクラという町に移っています。約500年前、ここから12000のも人が大西洋を渡ってヨーロッパ、アメリカ大陸に連れて行かれたそうです。自分の意志で生きられる今の環境と協力してくれる大切な人たちに感謝、感謝です！

I talked about next two years action plan with my colleague. Topics are management of daily work, making a list of discussion, recording of assessment, presentation at a conference and so on.She said that she want to hold study meeting. What is amazing is that she considers doing hydrotherapy if I teach them swimming. Is it true that all things will be implemented?

配属先の様子を現地の国営テレビ局が撮影に来ました。いつも30分やっている朝の集団体操を長くやってくれと言われ、約1時間に延長。さすがにネタが尽きました。
その後インタビューを受けて欲しいと頼まれ、拙い英語でスピーチ。どうかうまく編集してくれますように。

Local national TV came to take a film of my work place. They asked me to make longer group exercise than usual that takes 30 minutes. So I led it one hour. I finished ideas. After that I was asked to given an interview. I speech in English. I wish they edit nicely.

筋肉の延長術後は、ギプスでしばらく固定します。ガーナのギプスはかなりの重量があります。片足5kgはありそうです。脳性まひの女の子で、うまく意思表示ができないのですが相当ストレスだろうと思います。どこかから寄付されたウレタンで車いす改良。あとは周りの人が上手く使ってくれるかどうかが問題です・・
Dec 11th (162 days) setting of a wheelchair
After a surgery of extending muscle, we use plaster of palace. Ghanaian gypsum is heavy, it looks about 5kg for one leg. She is cerebral palsy, can’t indicate her intentions. I suppose that she has a lot of stress. So that, I arranged a wheelchair with urethane foam that donated from somewhere. The question is whether assistants can use it appropriately.

The TV crew has come to take a film during the week. Today, they took class for children and hydrotherapy that I was assigned. They took each and every scene. How long was it edited?
164日目、上腕切断
先週、右上腕切断の女性が2人入所しました。交通事故による骨折後の切断だそうです。作成したのは2本のワイヤーで肘と指を動かせる能動義手です。ガーナは左手を使わない文化の国なので、義手の操作を上達させないといけませんね。

Two women who have above elbow amputation cause of fracture by traffic accident admitted on last week. They were given artificial arm that is able to move elbow and fingers with active movement through two wires. They don’t use left hand in their culture, so that they have to improve control artificial arm.

167日目、重度障害
お母さんにおんぶされてきた重度四肢麻痺の女性20歳。アテトーゼタイプの脳性麻痺に似ています。痙性が強くて思うように手足を動かすことも自分で座ることもできません。標準型車いすを持っているようですが、使えていないとのこと。次回新しい車いすを処方するようです。ガーナではどこまで彼女のような人のための車いすの調整ができるのでしょうか？

A woman is 20 years old who has severe disabilities carried piggyback. It looks like Atheros’s type cerebral palsy. She can’t move her arms and legs, can’t sit herself because of spasticity. She has a wheelchair, but she can’t use it. Next time, she was given new one. How adjust a wheelchair for her in Ghana?

背中が痛いから診てほしいと頼まれました。左腕から指先にも痛みがはしるとのこと。頸椎と胸椎の境の関節が硬く、少しカーブしていて神経を圧迫しているようです。脊椎のモビライゼーションとマッサージ後、セルフストレッチを指導。これから週2回でPT継続です。

I was asked to see my colleague’s back pain. He has pain from arm to finger. His joint between cervical vertebras and thoracic vertebras are stiff and carved, it press a nerve. I did mobilization of skin massage, after that I instructed himself stretch. I continued treatment for him twice a week.

同僚とおそろいで作ってもらったワンピースが出来上がってきました。原色のグリーンをベースにした生地で普段はなかなか着ない色です。みんなで試着すると、まるで姉妹のようでした。クリスマスパーティーはこれで参加だそうです。

車のガソリンではなく、家のガスタンクが空になりました。ガーナでは空のタンクをガソリンスタンドに持って行って補充します。タクシー呼んで持って行こうかと思っていたら、配属先の人が交換してくれました。親切なスタッフに感謝。
Dec 19th (172 days) an empty tank
Not running out of gasoline, my gas tank was emptied. They bring an empty gas tank to gasoline station and fill up. I think to bring it by taxi, our staff changed for me. Thank you for my kind staff.

ガーナから手紙を送るのは初めて。封筒2セディ、はがき1セディでした。2週間以内には届くかな？
Dec 20th (171 days) Mail from Ghana
This is the first time to send a letter from Ghana. It costs 1 cedi for a post card, 2 cedis for a letter. I hope they are delivered in two weeks.
172日目、クリスマスプレゼントの準備
ガーナの布とピュアウォーター（ビニール袋入りの飲用水）の袋を使ったポーチ、ガーナの布で作ったフォトフレーム、色々なビーズを組み合わせて作ったピアス、などなど。包装紙で袋を作って、やっと完成！
Dec 21st (172 days) Preparation for Christmas presents
I made pouches that are made of Ghanaian cloth and bag of pure water (water for drinking), photo frame made of Ghanaian cloth, and pierced earrings made of several kinds of beads and so on. I completed them after made a bag with wrapping paper.

今はクリスマスの時期、歌番組が放送されていました。大人数のコーラスグループ、コーラスとメインボーカル、6人のアカペラグループ。多くのグループが歌います。出演者はプロではなくおそらく一般の人たち。教会礼拝で普段から歌っているだけあって、みなさんゴスペルソングはかなりの腕前です。

It is a Christmas season now. A song show was broadcasted. A great number of chorus group, main vocal and chorus group, a cappella group and more. Casts may be all amateur. They had good abilities of gospel song, because they usually sing a song in a church service.

クリスマス休暇に向けて多くの人が退院します。退院の目途が立った子ども達はみんな嬉しそうな顔をしています。誰しもやっぱりおうちが一番。

Many patients will be discharged in Christmas holiday. All children who have hope of discharge are looking happy. My home is best for everyone!
175日目、ちょっとずつ北へ
モレナショナルパークに向けて出発。今日は途中のクマシという街まで移動です。乗り合いバスはどれも満席で、現地の人の車に乗せてもらいバス停へ。クリスマスイブで大渋滞の中、シニア隊員のお宅に到着。窓から花火が見えました！
Dec 24th (175 days) Going to north little by little
I leave to Mole national park. I go to Kumasi on the way to goal. All buses were full, so that local people brought me to bigger bus stop. I arrived at senior volunteer's house. I saw fireworks through a window！
176日目、クリスマスパーティー＠タマレ
約15人の隊員が一人の家に集まりました。ホロホロ鳥を絞めて、バーベキュー。ガーナ人の主食の一つＴＺを初めて食べました。他には 鍋、手作りお菓子など。スイカ割りをして、カラオケ大会！楽しい会を準備してくれたみなさんありがとう。
Dec 25th (175 days) Christmas party in Tamale
20 people gathered at one of volunteer’s house. We tied and cut a guinea fowl, then cook barbeque. After that, I had TZ that Ghanaians eat usually. The others were, Nabe, handmade cakes. Then, bit with a bar and divided a water melon. Then, sang a song with a karaoke set. Thank you for preparing happy party!

同期隊員と一つ前の隊員と11人でバスをチャーターして、モレ国立公園に向けて出発！ジープの屋根の上に設置されたベンチに座り、サバンナの中へ。15分すると車から降りるように言われ、呼ばれた方向に行くと、大きな象が目の前に！野生の姿に感動です。
Dec 26th (177days) Wild life
We chartered a bus and left Tamale to Mole National park with 11 volunteers contemporaries at training center and volunteers who trained previous term!Sat on a bench that had set over the roof of Jeep and moved through a savanna. After 15minute, we were told to get down, and then I walked to the direction that a guide said. There was a big elephant under the tree before my eyes. I was deeply affected from a true picture of wild life.

今日はウォーキング、鹿、猛禽類を見ながらあるくこと30分。広い大地をゆっくり歩く象に遭遇、2日とも象に会えるなんて嬉しい!!

We went walking for 30 minutes, seeing deer and raptors. .Suddenly, we met an elephant that was walking slowly. We are lucky because had seen elephant twice for two days.

新幹線のないガーナの人たちの主な移動手段は車です。主要都市間は長距離バスが走ってます。STCバスでタマレからクマシという都市まで 5－6時間、30GHC約1500円です。エアコンがきいて、ゆったりしたシート、まるで日本のバスみたいに快適！

A means of transportation for Ghanaians who don’t have a limited express are vehicles. A long-distance bus runs between main cities. It takes 5-6 hours by STC-bus and costs 30GHC (1500 Yen). It was comfortable and looks like Japanese bus, because it is a fully air-conditioned and has spacious seats.
180日目、洗濯
久しぶりに自宅に戻り、土まみれの服の洗濯に取り掛かりました。3回洗ってもまだ水が黒い・・乾燥と暑さと土ほこりの中よく頑張ったわ。
Dec 29th (180 days) washing
I returned home after a long absence, I had started on washing of muddy clothes. Water after washing same trousers 3 times is still dirty. We had well done in spite of dry and heat and dust.
181日目、ホテル整備
年内最後のマーケットデーに買い出しに行きました。地元のマーケットで今までで一番大きなお買いものをしました。お客さん用のマットレスを35セディで購入。これで最大2人我が家にお泊めできるようになりました。
Dec 30th (181 days) preparation of accommodation
I went shopping on market day in the end of a year. I bought the most expensive item in a local market. I paid 35GHC for a mattress for a guest.
