支柱と靴は足底で接合されている。足にとって良いのかしら・・近々考えようっと。
49日目、X線読影
交通事故後1ヶ月経ってまだ肩が痛いという男性が外来に来ました。副ダイレクターによると、X線上で大結節の剥離骨折と腰部の神経根の狭窄があるとのこと。病院からのレポートには腰椎に関して「所見なし」と書いてありました。
まだガーナでは放射線技師のレベルはまちまちなのかも知れません。

X-ray interpretation
A man who has shoulder pain by trafic accident one year ago came for consultation. According to a vice director, there is an avulsion flacture of Greater tubercle and a nerve root of lumbar is constrict on X-ray. The report of a hospital wrote "No findings" about lumbar vertebra.
The level of radiologist may be devided in Ghana.
50日目、子どもたちの朝の体操
子どもたちは平日は、1日3回のマット運動をしています。今日は自分が担当させてもらいました。いつもやっているプログラムにいくつか加えてみました。スタッフのように厳しくコントロールするのはまだ抵抗がありますが、ガーナの慣習に習わないといけませんね。
10mの歩行スピードと歩数を計るための計測走路がありません。というわけで定規で10mを計測しました。最初の患者さんは脊椎症で腰痛と下肢痛の外来患者さん。今度はTimed Up and Goを試してみようと思います。

Morning exercises for children
Children do mat exercises 3 times a day on weekday. I teached them exercises today. I added a few exercises in a general program. I don't feel like handling children strictly, I have to do follow national customs in Ghana.
There is no lane to measure the time of 10m walking and steps. So that I measured 10m with a ruler. The first patient is outpatient who has lowback and legs pain cause of spondylopathy. I want to try measuring Timed Up and GO on next time.
51日目、ガーナ人が義肢装具士になるには
ガーナの義肢装具士は国家資格です。しかし、現在国内には養成校がなく、海外で勉強したあと戻ってきてガーナの国家試験を受けます。
2週間後に配属先が運営する国内で初めての義肢装具士養成校が開校します。学位が取れる3年間のコースとのことです。ここは教育機関ではないけれど、臨床的な勉強ができるよい環境だと思います。

How to be a Prothesist and Orthosist for Ghanaian
In Ghana,a qualification of Prothesist and Orthosist is a national certification. But, there is no training school in this country, they have to take an national examination in Ghana after studied in some foregin countries.
The first training school of Prothesist and Orthosist in this country managed by my work place will be open 2 weeks later. This course takes three years to take a diploma. This is not an educational insutitution but I think this school has a good environment because they study clinically.
52日目、プールエクササイズ？
私の配属先にはプールがあります。5m×7mくらいの小さな屋外プールですが。半分が水深1mもう半分は水深1.5mくらいあり、私の足がつかないくらいです。同僚からのリクエストでプールに入ることになりました。同時に子どもたち2人ずつに入ってもらいましたが、制止するのが大変です。同時に溺れないように見ていないといけないのでそれもまた気を遣います。
勝手に泳ぎだす子もいるので、やはりもっと厳しく接しないといけませんね。。

Aqua exercise?
My work place has a pool, but It is a small pool. It is 5m in width and 7m long. Half of the pool is 1m deep and the other side is about 1.5m deep, it beyond my depth. It turned out that I do aqua exercise by my coworker's request.I asked them to enter the water two children at a time, but it is heavy work to control children. I have to be careful about they don't drown.
One of them start to swim as one likes, after all, I should be strict with children..
53日目、装具事情
配属先の製作室では義足、整形靴、膝装具と靴を合体させたキャリパー、松葉杖、座位保持椅子を製作しています。ですが、プラスチック装具は作っていません。しかし、海外から寄付された中古品が山のようにあります。
先週対応した右片麻痺の年配の女性にプラスチックAFOを試すことにしました。でも、棚の中はぐちゃぐちゃで、左足用、小さい、長すぎる、ベルトがない、トリミングが深すぎるなどなど、種類はあっても必要なものが見つかりません。
ふと外を見ると、義肢装具士の学校開設に向けて、数人がいろいろな物品を運んでいました。置かれていた段ボール箱の中にも体幹装具や下肢装具がたくさんありました。中を見ると欲しいサイズの装具があったので次回はこれで試してみようと思います。

Orthopedics condition
In the workshop of my workplace, they make prothesises, orthopedic shoes, caliper that is combined of knee brace and shoes, crutches, seating systems. But, they make no plastic orthosis. However, There are mountains of used orthosis donated from abroad.
I tried plastic AFO to an old lady whom I treated last week. But, inside of shelf was mushy, one orthosis is for left leg, one is too small, too long, no belt, too much trimming, so that I couldn't find a item that I need.
Untentionally, I looked outside, a few people carried some items for opening of orthosist school. There are lots of spinal orthosises and lower limb orthosises in a corrugated fiberboard box. I sow inside of the box, I found a orthosis which size is that I need, so that I will try with this one.
54日目、トロトロ
ガーナ人の足としてよく使われているのが「トロトロ」です。日本でいう路線バスみたいなものですが、ボロいワンボックスカーで、時刻表はありません。満員にならないと発車しません。バス停の名前はあっても、日本のように看板？はありません。ドアの開閉は「メイツ」と呼ばれるお兄ちゃんが手動で行います。集金もこのお兄ちゃんが一人ひとり行います、いわば車掌さんです。
今日は任地から首都に戻るため、途中の町で一度乗り継ぎをしました。今日は、道が混んでいてバスが来ない、そしてなかなか満員にならなかったため乗継のために1時間くらい足止めされました。この国では時間に余裕を持たないと生きていけませんね。

Trotro
"Trortro" is a standard means of transportation for ardinaly people in Ghana. It is what called a bus on a regular route in Japan, an old tattered minivan, it has no timetable. It doesn't leave until every seat was filled. There are bus stops but almost all of them have no signs. A young man called "mate" opens and closes the door by hand. He collects money one by one, as it were a conductor.
I changed buses at the station along the way to capital. Today, the bus was taking a lomg time to arrive because of a traffic jam, so I stayed there for one hour because seats until seats became full. I can't live in this country if I have limited time.
55日目、日本食
今日は保健分野の隊員の集まりでボランティア調整員さんのお宅にお邪魔しました。ワークショップの前に、日本食を堪能しました。私の住んでいる村には生で食べられる魚は売っていないので、お刺身を食べるのはガーナに来て以来初めてです。お米はあきたこまちです。久々の手巻き寿司は本当においしかったです。
先輩隊員の作った、手洗いを推奨する劇と現地で人気のある歌を替え歌したものを見せてもらいました。みんなで楽しめるものはいいですね。

I visited a house of volunteer coordinator of the meeting for volunteer who were assigned to health field. Befor workshop, we were fully satisfied with Japanese food. I can't gat raw fish in my village, this is first time to ate sashimi since came to Ghana. Name of rice is "akitakomachi". Hnad-rolled sushi was really delicious.
I sow a short play and a parody of a song that recommend hand wash made by volunteers. It was nice performance that have a good time together in a group.
56日目、総会1日目
年に2回の隊員総会のため、朝から貸し切りバスに乗り込み、ガーナ大学内の野口記念講堂に移動しました。広くて緑豊かなキャンパスです。内容は各種講座と先輩隊員の活動報告などです。 学校教育や保健活動の報告、職業訓練校における理学療法士の報告、プログラムオフィサーによる、シアバターを作る女性クループ、加工業者、販売業者のコーディネートの話です。
みなさん工夫を凝らした活動をしていて、感心されられると同時に気が引き締まりました。自分も何か探してトライしないとなぁ。

1st day of general assembly
We moved Noguchi memorial auditrium in Ghana university by a chartered bus to join a general assembly that hold twice a year. The campus robed in verdure. Contents were some lecture, presentation, and so on. A report on the preceding year's activities by phisical therapyist, coordinate of women's group who make sia butter, a processor and a sales company.
I was impresed that everyone worked with good idea and I was feeling of tension and determination. I have to try something that I can.
57日目、総会2日目と大そうじ
2日目は、教育、保健、公務員研修グループに分かれて、それぞれの分野の中でガーナが抱える課題について話し合いました。
そのあとは半年に1度行うドミトリーの大掃除。マットを天日干し、冷蔵庫の清掃、物品整理、などなど。
洋服、山のようなDVD、文房具、ラケット、琴まで！あらゆるものが出てくる出てくる・・ やっぱり家がきれいになると気持ちいい!

2nd day of general assembly and a general house cleaning
In the 2nd day, We are divided into three groups those are education, health and training for sivil servants, then discussed about issues which the Ghananian Government has in each group.
After that, We did a general house cleaning of our dormitory once a half year. sun drying of a mattress, cleaning of a flidge, rearrangeing goods and so on.
Every conceivable items came out, clothes, moutains of DVDs, stationelies, a racket and a koto! After all, it's comfortable to clean a home!
58日目、女子会
JOCVではない人との会話からたくさん情報をもらえました。ガーナ大学の様子、ソーシャルワーカーの隊員が以前障害分野に関連した活動をしていたという話。知り合いに紹介してもらったガーナに住む女性たちです。皆さん最近ガーナに来たばかりなので、「ガーナの人ってあまり強く主張したりしないね。」とか「アクラにいると食費がかかるよね。」とガーナの印象を語り合ってきました。
いろいろな人とつながって、お互いに助け合えるといいですね。

Girl's meeting
I got plenty of information from conversation with people except JOCV. How is the University of Ghana, about one of old JOCV as a social worker who did activities related to field of people with disabled. They are women whom I met through introduction of my contacts.
I hope connect many people and help each other.
59日目、1年報告会
活動をはじめて1年が過ぎた隊員による1年報告会が開かれました。理学療法士隊員からは、手術を勧めて歩けるようになった生徒のエピソード、部屋の環境整備、理学療法アシスタントを育てるという今後の課題。そして、これまでの経過を聞くことができました。
教育分野の隊員からは、どうしても課題を見つけて改善しようと考えてしまうけれど、良いところを見つけて誉めたうえで提案していくと良いといった案が報告されていました。教材室を作ったところ先生同士が会話する機会が増えたという報告もありました。1年後に自分がどれだけ活動できているかちょっと心配です。1年後に報告することを想像しながら日々を過ごすかな。

An annual briefing session
An annual briefing session was hold by volunteers who did activities for 1 year. A physical therapist reported some episodes about student who can walk after recomendation to have surgery, putting a room in order, a task for the future that training physical therapy assistant.In addition to that,I heard stories till now.
I knew other reports by volunteers whose field is education, we are apt to point out and improve problems, but it is better to suggest somothing besides point out and praises their good point. Another one reported that opportunities of conversation between teachers increased after make teaching materials room. I warry about how I will be able to do. I want to spend everyday while thinking something that I will report one year later.
60日目、1週間ぶりのお仕事
1週間前に下肢装具を試した右片麻痺の女性がやってきました。どこかからか寄付されたプラスチックAFOを合わせたらまずまずフィットしました。でもアンクルベルトがありません。仕方ないので弾性包帯で固定。自宅で試してもらうことにしました。あとは右上肢の浮腫と痛みを軽くしてあげたいです。
他には変形性膝関節症の女性2名が来ました。角度と筋力を測ってメモに取る、そして自分で保管。今度カルテに書き込んでいいかどうか聞いてみようっと。

Work for the first time in one week
A right hemiplegia lady I tried AFO for her last week came. One of the plastic AFO that donated from somewhere fit her leg well. But there is no ankle belt. Helplessly, I strapped with a bandage. I let her use it in her home. And then, I want reduce edema and pain of her right arm.
After that, two ladies who have osteoarthritis of knee joint came. I measured angle and muscle strength, took a note, stocked myself. Next time, I will ask if I may put it down in a patient's case record or not.
