FS2004 - Ghost Plane (Helios Airways Flight 522)
the previous flight crew had reported a frozen door seal and abnormal noises coming from the right aft service door.
They requested a full inspection of the door.The inspection was carried out by a groung engineer who then performed a pressurization leak check.
In oeder to carry out this chck wituout requiring the aircraft's engines,the pressureisation system was sent to "manual".
However,the engineer failed to reset to "auto" on completion of the test.

I can't even imagine the terror Andreas Prodromou has suffered. They all put on their oxygen masks without even knowing that the pilots
 are already out. That gave them around 20-30 minutes of oxygen and then they started too passing out. Then, after a while, Andreas managed
 to find consciousness again because he was a free diver and started using all the oxygen cylinders he could find in the airplane among
all those unconscious passengers and crew only to eventually find out that the pilots had the same fate. Then, since he was a qualified pilot,
 he tried to land the plane while trying to communicate with the F-16s, though he couldn't because the radio was still connected with Larnaca
's airport. It was too late, there was no fuel left. He was a true hero battling with death itself. He changed his schedule the last minute
to spend some time with his girlfriend, a flight attendant on the same plane.

4:36 apparently the pilot was already under the influence of hypoxia at this point which is why he asked this question instead of answering

This interesting video fails to explain that the subsequent investigation revealed the captain was to blame as it's on his check list to
 ensure the pressurisation switch is at auto. Also, the flight attendant seen moving had aspirations to become a commercial pilot and was
working on his private pilots licence.  He therefore had the knowledge to recognise the onset of hypoxia and used the small emergency oxygen
 cylinder to stay alive.  It seems he intended to try to bring the plane under control.  The saying of someone being on a high comes
from hypoxia as you get a feeling of well being in the early stages of it giving a similar affect as some drugs.
Overall, a terrible tragedy.
