巨大結腸の原因は、
正常であれば腸管内の Auerbach 神経節細胞が胎生6週頃に移動し、胎生12週頃までに直腸の最下端部に到達するが、エンドセリン 3 (EDN3)あるいは EDN3 の受容体であるEDNRBの変異により、種々の程度で神経節を欠如し、胎便排泄遅延を起こし、巨大結腸を呈する。
