This is ruled by law.This is a part of the application form downloaded from Yamagashi-shi website【Where can I get it?】At the local government where you registered your address.【How much does it cost?】It depends on the city. Approximately ¥150-300【What to bring to there?】・Indentification of the person who will do the paperwork It depends on your jurisdiction but the procedure would be easier when you show photo ID. The best ID is photo ID, such as a residence card, driver's license or passport (foreign passport is OK). If the person doing the paperwork doesn't have photo ID, you might have to do extra documents that will take some time. I strongly recommend bringing photo ID.・An Attorney letter if you are going to on behalf of someone【When is the best time to go there?】Many local government offices open from 8:30 -17:15 on weekdays. Some are open until 7pm on weekdays and 8:30-17:15 on weekends. It depends. You can check the hours of your local office online.Monday morning and 4-5pm tend to be busy. Around 11-1pm are also busy because there may be not enough staff since they take turn to have a lunch break.【How to fill in the application?】This is an example application form downloaded from Yamaga-shi website(山鹿市). I wrote A,B on it so that I can explain easier.This is the application form for Copy of Koseki (戸籍 - Family registration, exclusive to those who have Japanese nationality), Juminhyo(住民票 Residence ), Inkan(印鑑 ). - ①Fill in your information↑The blue sentence above means ※You'll be required to show your Photo ID for identidication bring one such as Driver's license, My number card, Passport, or Handicapped person's passbookThis is the application form for Copy of Koseki (戸籍 - Family registration, exclusive to those who have Japanese nationality), Juminhyo(住民票 Residence ), Inkan(印鑑 ).You have to write down the following information・氏名(しめい)=名前 Your name - Write it exactly as it is on your residence card The order should be Surname, First name, Middle name.・住所(じゅうしょ) Address - You can just write the City. Prefecture isn't needed.・生年月日(せいねんがっぴ) Date of birth (check whether to write in Japanese or Western style) - ②Purpose of use etc※About 'the Purpose of useAs I explained earlier, There are essential and optional information.When you apply for the Juminhyo, You have to choose the information you want on it such as The name of representative of household, Relationship with household, your nationality etc.Whether they are essential or not, that depends where you're using your Juminhyo. For example, if you register your car, you won't need The name of representative of household etc. They are not relevant to the process you are going through, so just don't make a check. When you need one for renewing your visa, you should make checks all the information except for My Number.The staff will ask you the reason you need a Juminhyo so they can guide you with checking the boxes.If you are unsure, I strongly recommend you ask the person you are submitting the Juminhyo to. - ③Select 個人番号 / 世帯 / 外国人証明For example, when you apply for Juminhyo, you choose whicheverFrom Section C・ List just one person's information or a whole household If you need to show your whole household members, select 世帯全(部)員(This is sometimes written 謄本) If not, select 世帯一部(This sometimes goes by 抄本) If you need this copy for renewing visa, or applying for new visa, I recommend selecting 世帯全員From Section D・Make a check if you want them on a Juminhyo個人番号(Mynumber)… DO NOT put a check here unless MyNumber is required (in cases such as renewing your visa)住民票コード(Resident register code)… this is usually used only at national institutions. You most likely don't need it.From Section E・If you need this copy for renewing your visa, or applying for a new visa, you should place a check in all of these boxes.【When you're done】Submit this application form at the 市民課 (Shimin-ka) reception. The staff will give you a number slip, and the number will be called when it's ready.When you receive your Juminhyo, take your time there and check that there's no mistake with your personal information. Especially when you go during busy times, you might get a document with a mistake or two!
