What's the difference between Juminhyo(住民票) and  Kisaijiko Shomei (記載事項証明書)?

They are basically the same. 

 

Juminhyo proves your:

・Address

・Name

・Date of birth

・Sex ※

・The date of becoming a citizen of the city

・The date when the address was determined (Notification date)

・Previous address

 

 

And the information below. There are optional (For non-Japanese people)

・The name of representatibe of household

・Relationship with household

・Nationality

・Period of stay, qualification of staying and expiry date

・The number on a residence card

・My number

・Resident register code

This is the Sample of residence certificate downloaded from Kobeshi Chyuo ward. Yours might look a bit different, but the information you will need to provide is the same.

 

On the other hand, Kisaijiko is like a symplified document of Juminhyo. The points are

 

①The Items written with bold above, does not included

②Sex is also optional

 

I couldn't find a good sample of Juminhyo kisaijiko, sorry!
