こんにちは。
今日は、採卵周期開始のためにクリニックに行って来ました。
前回のアポで採卵周期開始予定だったけれども、排卵がまだだったので注射で排卵を起こしました。今日は、排卵確認と採卵周期開始のためにクリニックに行って来ました。
この日は、会社をお休みせずに通院。前の病院では、午前休みor午後休みを取らないと通院できなかったので、会社を休まずに通院できるのはありがたい。
とはいえ、定時きっかりに出社してダッシュで病院へ。
早く帰りたい日に限って、打ち合わせが多い。。朝からその日の仕事・打ち合わせを計画的に組みサクサクと終わらせる。どれか一つでも上手くいかないとクリニックに間に合わないのでどれも集中。。
6:00 pm アポ。採血があるため30分前に到着。
5:30 pm チェックイン
5:30pm 採血一本
5:40 pm 内診
内膜の厚さ、卵胞数確認
6:20 pm 診察
先生から下記説明を受ける。
-排卵済み。（内診時に見えた大きい卵胞40mmくらいは排卵済みの卵胞とのこと）
-小さい卵胞が左右合わせて10個あるからこれを育てていきたい
-血液検査結果値のホルモン値はよし
-注射で摂取する薬の説明
（前のクリニックで、卵胞の育ちが悪かったことを話したら、薬の量を増やすことに）
-今日は２種類の注射を院内でうち（フェリング300、エナルモン1A）、明日から自己注射開始（フェリング300）
-次回のアポ日と採卵予定日
一通り説明を受けたのち、用意していた質問を聞いてみる。
-刺激法、受精方法
（前回のアポ時に渡された採卵前スケジュールには、刺激法＝黄体フィーとバック法、受精方法＝Split ICSIと書いてあったので、理由を聞いてみる。）
刺激法＝黄体フィードバック法ではなく、ランダムスタート法の間違いとのこと。その場で先生が、オンラインカルテを修正。（言わないと間違えた刺激方法になっていたかと思うと恐ろしい。。）
刺激法＝Split ICSIは体外授精と顕微授精に分けて行う方法とのこと。今までは顕微授精のみしかやってこなかったことを伝えたら、顕微授精に変更。卵子活性化法もやって胚盤胞まで行ったことを伝えたらこれも追加。受精方法は採卵時まで変更可能とのこと。（言わないとクリニックのオーソドックスな方法に自動的になる模様。。）
-慢性子宮内膜炎の検査結果
２週間前に行った、慢性子宮内膜炎の検査結果をまだ聞いていなかったので、どうだったか聞いたら、「あれ、渡していませんでしたっけ」と。
前回のアポでは、ランダムスタート法では採卵時の慢性子宮内膜炎検査できないと言われたので、陽性を覚悟。そしたら、陰性とのことで、結果を渡されました。
6:40 pm 院内注射
看護師さんから今日打つ注射の説明と、持ち帰る自己注射の説明を受ける。
-注射を打つタイミングはいつでもOK
-薬剤２瓶と溶解水１本
-針は２種類あり、ピンクのカバーがついた針で注射液を作り、グレーのカバーがついた針で腹部に刺すとのこと。
-45度の角度で打つのを推奨とのこと。
-看護師さんによるフェリング注射の実演（お腹をつまんで、45度の角度から刺す。奥まで針が入ったら注入。）
-打つ時のポイント
おヘソよりも下の方が痛くない
痛点に当たると痛いけれども、当たらなければ痛くない（経験でわかってくる）
電気が走るような痛みがあった場合は、刺す場所を変えること
フェリング注射は、染みる痛さがあるとのこと
エナルモン1A注射はそこまで痛くないとのこと
-確かに、フェリングは刺されている間ずっと痛いけれども、エナルモン1Aは刺される時の痛みはあったけれども、注入の痛みはなし。
7:00 pm 会計
採血から１時間半で終了。テキパキ進むのはありがたい。仕事を休まなくていいのもありがたい。

-聞きたいことは聞く
-聞くことを忘れそうだったらメモを用意
-カルテに書いてあるから分かってくれてるだろうと思っても、口頭で伝えることは大切（相手は意外と分かってない。仕事でも同じ）

Hello,
I visited Reproduction Clinic Tokyo today to start ovum collection term.
I had expected to start this term on the previous appointment day.
Unfortunately, ovulation didn’t occur and was postponed to today.
Today, luckily I didn’t take off from my work. However, I left my office on the time to finish my work and rushed to the clinic.
To leave my office exactly on time, I finished my works and meetings very efficiently from this morning.
My appointment was 6:00pm. I arrived at the clinic 30min. before the appointment to have blood test.
5:30pm Checked-in @Reception
5:30pm Had my blood collected @Blood collection room
5:40pm Checked my thickness of endometrium and number of eggs to grow @Internal examination room
6:20pm Consulted with a doctor @Consultation room
The doctor told me the following points.
-Ovulation did happen
-Hormone level from blood test is good
-There are 10 expected eggs
-Explanation of injections
have 2 injections today at the clinic(HMG injection 150IU "Ferring” and Enarmon1A)
have 1 injection everyday from tomorrow at home(HMG injection 150IU "Ferring”)
-Next appointment day to come
After the explanation, I asked a few questions I prepared.
-Stimulation method and fertilization method
My stimulation method was “Luteal feedback method” and fertilization method was split ICSI. I asked the doctor about these choices.
The doctor said my stimulation method is “Random start method” not “Luteal feedback method”. The form I got during the previous appointment was wrong. The doctor corrected the records on the screen.
As for the fertilization method, Split ICSI is the combination of vitro fertilization and micro sense fertilization. He told me I can decide the method on the egg collection day. At the previous clinic I always choose micro sense fertilization to make it happen. I requested to change it to all micro sense fertilization with ovum activation method.
-Result of Chronic Endometritis Examination(BCE)
I took BCE 2 weeks before and asked for the result, which turned out negative. The doctor told me he had already given me the result.
6:40pm Got 2 injections @Treatment room
-A nurse explained what I will take today and from tomorrow.
-Showed me how to get injection and I had two injections
-No specific time to take
-Two medicined bottle and one water bottle to be mixed
-Two needles are packed for one injection(One for making the injection solution. The other is for injecting to the body)
-The angle between the needle and body is to be 45 degree.
-Tips to have injection
the part to be injected should be under belly button
If the needle touched the pain point, I will feel pain
If there is an electronic pain, change the point to be injected
The injection of “Ferring” will make a pain
7:00pm Paid bill @Reception
Very smooth process to finish all process within 1 and half hours.
The business skills which worked today during the process.
-Ask and make sure what the doctor says and said
-Be prepared for the consultation
-Tell to make sure the doctor really understand my situation
