今回は『一過性意識障害』の患者における『舌咬傷』の見方について少し。

一過性意識障害を主訴に救急外来を受診(救急搬送)されるケースは非常に多いですよね。でもそれが果たして本当に『失神』なのか『てんかん』なのか、それ以外の『一過性意識障害』を来たす疾患なのか悩ましいことも多々あります。(以前にブログ内でお話させて頂いた『失神の鑑別』を参考にしてください。)
病歴が全く不明で、誰も失神の目撃者がいない。そんな時でも身体所見で鑑別の役に立つのが『どこに舌咬傷があるか』ということです。
こんな風に舌縁に咬傷をみつけたら『てんかん』を疑ってください。咬傷が軽度である場合は患者本人も気がつかないこともあります。きちんと所見がないかとることが大事になります。
一般的に全身性発作の20-30%程度に認められ、強直発作の時に起こると言われています。咬傷がないからといって『てんかん』の可能性を否定することはできませんが、あれば疑うことができます。
一過性意識障害の時に舌咬傷のような傷を負うような場合、他にも手を怪我したり後頭部に傷を負ったりというような場合は精神的な『Psychogenic nonepileptic seizures(pseudoseizures)』では起こらないと一般的には考えられています
しかしながら救急外来に何度も『Psychogeinc seizure』で受診するようなベテランさんは我々救急医を欺く術も知っています。
私がERで働いていた時には発作の時にSpO2が80台になるまで頑張るつわもの?もいました。発作の時の脳波がとれれば確実なのですがなかなか難しいですよね。
『Psychogenic seizure』を身体所見で見分けるポイントはいくつかあります。良く観察しているととても不自然な箇所がいくつも見つけられるのです。(意識や目の動き(または目を閉じたり)、発作の様子(非同期の動きであったり、首を左右に振ったり)、発作後の様子など。)ここにpseudoseizureの動画もあります。非同期的な動きで自転車を漕いでいるみたいです。

Selected Differential Diagnosis of Tongue Lacerations

Sudden onset of a focal neurologic deficit lasting a few minutes to a few hours; abrupt loss of consciousness is not a focal symptom and is not suggestive of an attack.

Usually have prodromes and do not often cause tongue lacerations; if the tongue is bitten, it is on the tip rather than the side.

Epileptic seizures |
Generalized tonic-clonic (“grand mal”) seizures cause lateral tongue lacerations; typical sequence is body stiffening (tonic) followed by generalized jerking (clonic) lasting one to two minutes, followed by a deep sleep (postictal phase).

Psychogenic seizures |
Can vary but usually do not cause any type of injuries; tongue biting is rare.

Hypoglycemia episodes |
Usually abundant prodromes of malaise, fatigue, dizziness; tongue biting is rare. |
