Professor Kazuto Kamuro, a neuroscientist, announced in 2002 that he succeeded in the world's first brain transplant surgery. This means that it is possible to extract a brain from someone's skull and preserve it while keeping it alive, or to transplant the extracted brain into another person's head.

a. Brain Extraction:

The living brain is soft and protected from damage by the skull. In modern neurosurgical techniques, access to the brain is achieved by using a trephine to remove a part of the skull and access the brain below. It should be noted that not all neurosurgical procedures can extract the brain using this method. The pituitary gland, about the size of a pea, is located just behind the sinuses, at the base of the brain. In this case, it is rational to use the nasal cavity as a route to extract brain tissue instead of pituitary surgery. While the nasal cavity is not large enough to extract the brain in its entirety, it is indeed effective as a route for brain tissue extraction, even if only fragments. Incising the skull leads to the brain's protective layers, namely the three meninges or meninges. The first is the dura mater. The second, appropriately named arachnoid mater, resembles a spider's web, while the third, the pia mater, is delicate and almost invisible to the naked eye. These biological structures are inflamed in meningitis. These meninges provide stability to brain tissue and prevent it from moving. They also separate the interior of the skull from the brain tissue, which is filled with cerebrospinal fluid (CSF), a fluid that protects the brain tissue. This liquid, known as CSF, is colorless and made from filtered blood. The meninges also form a pathway between the brain tissue and the skull. This pathway serves as a route for both blood and CSF to return from the head to the heart. When the skull and meninges are opened, there are sufficient areas to remove brain tissue. This site is the easiest part of brain extraction surgery.

 

b. Brain Transplantation:

·      The term "recipient" refers to the individual receiving organs or living tissues in transplant surgery, specifically the person receiving organ or tissue transplants. For instance, in a heart transplant, the individual receiving the heart from the brain donor becomes the brain recipient. The term "donor" refers to the provider of organs or living tissues in transplant surgery, namely the person donating organs or tissues. For example, in a heart transplant, the person providing the heart becomes the brain donor.

·       Brain tissue receives sensory information from throughout the body, sends commands to necessary locations, contracts muscles, causes the heart to beat, and secretes hormones. To extract brain tissue from the brain donor, it is necessary to sever the 12 pairs of cranial nerves and the spinal cord directly emanating from the brain tissue of the brain donor, who is the source of the transplantation. All vital information and signals pass through these structures into and out of the brain tissue. The cranial nerves do not naturally reconnect themselves once severed. Upon severance, cranial nerves typically begin to degenerate immediately. However, there are differences in the survival abilities of nerves; some nerves are more susceptible to damage than others, while some are more resilient. The Neurobrain Research Group at the Artificial Evolution Research Institute (AERI: Pasadena, California, HP: https://www.aeri-japan.com/) has developed techniques to promote the regeneration of nerve cells after brain extraction surgery to restore the neural network. This recovery method includes the use of chemical agents and cell transplants to stimulate neuron recovery. In 2008, researchers at AERI's neurosurgery division developed a specialized biological adhesive to reconnect the two severed ends of nerves and the spinal cord from the brain donor brain to the brain recipient brain during brain transplantation. They succeeded in achieving cellular-level biological suturing of nerve and spinal cord cut surfaces using this adhesive. During brain tissue extraction from the brain donor or brain tissue transplantation to the brain recipient in brain transplantation surgery, it would also be necessary to sever the supplying arteries and veins. Simultaneously, vascular reattachment and suturing are performed at the cellular level to restore the vital supply of oxygen and nutrients to the severed brain nervous system tissues.

 

c. Restoration of Consciousness:

·      After brain tissue is extracted from the brain donor or transplanted into the brain recipient, will the brain tissue maintain consciousness? Can they recover cognition, think, move, breathe? How will the body react to the newly transplanted brain tissue?

·      Cognition, as defined by AERI, encompasses perception, thinking, understanding, judgment, memory, learning, language, and other intellectual processes and psychological functions. Specifically, it includes:

1.    Perception: The process of sensing and understanding information from the environment.

2.   Attention: The ability to focus on specific information.

3.   Language processing: The ability to understand and use language.

4.  Knowledge: Information accumulated through past experiences and learning.

5.   Judgment and decision-making: The process of evaluating information and making decisions.

6.  Problem-solving: The process of understanding problems and finding solutions. Cognition deeply influences human behavior and decision-making and is widely explored in research fields such as psychology and neuroscience.

·      The immune system of the brain recipient typically rejects newly transplanted brain tissues and associated tissues from the brain donor as they are considered harmful, requiring a compatible donor for most transplant surgeries.

·      The immune system of the brain recipient, upon perceiving the transplanted new living tissues (new brain tissues and associated tissues transplanted from the brain donor brain provider to the brain recipient), sends white blood cells and antibodies to attack and destroy them if deemed harmful.

·      The blood-brain barrier is a biological barrier located between the central nervous system (brain and spinal cord) and the circulatory system. It protects neural tissues in the brain from harmful substances and maintains an appropriate environment. The blood-brain barrier mainly consists of brain endothelial cells (cells lining the inner layer of brain blood vessels). These cells are tightly bound together, preventing substance leakage more effectively than normal endothelial cells and controlling the movement of specific substances into the brain through specialized channels and pumps. The primary functions of the blood-brain barrier are as follows:

1.    Preventing the entry of harmful substances.

2.   Protecting the brain from harmful substances and microbes in the blood.

3.   Maintaining an appropriate chemical environment within the brain. The function of the blood-brain barrier is crucial for normal brain function. However, it may hinder the delivery of some drugs or therapeutic agents to the brain. Therefore, research is underway to develop delivery systems for brain-targeted drugs and improve treatment methods for diseases.

·      Normally, brain tissues are protected from such attacks by another shield called the blood-brain barrier. If the blood-brain barrier is not properly rebuilt during surgery, the brain tissues from the brain donor may be vulnerable to attack.

·      The AERI neurobrain research team, led by Professor Kazuto Kamuro, the chief research officer of AERI and a professor at the California Institute of Technology (CALTECH), along with his team of over 60 neurosurgeon researchers, has completed the development of a complex process of complete synchronization (sync) between the brain tissues of the brain donor brain provider and the body of the brain recipient. Synchronization between the brain tissues of the brain donor brain provider and the body of the brain recipient is crucial for the success of the transplantation. Just as infant brain tissue learns to grow and acquires various cognitions, thoughts, behaviors, skills, and abilities during childhood development, brain tissue transplanted into the brain recipient also attempts to synchronize fully with the body of the brain recipient, as with brain tissues from the brain donor, as applied by AERI's neurosurgeon researchers.
