ピンク・フロイドの1973年のアルバム『狂気 The Dark Side of the Moon』に収録されている作品。
曲の中でコンセプトである狂気と「月の暗い側面」を結びつけた、本アルバムの核となる作品。曲の始まりは少々デヴィッド・ボウイの『スペース・オディティ Space Oddity』（1969）を連想させるところがあるが、ドラマティックな展開とサビはいかにもピンク・フロイドらしい。
「メスを取り上げ云々」という語句は脳外科手術を示唆している。現代の感覚からすれば精神疾患と脳外科手術とは結びつきにくいが、1960年代までは前頭葉白質切截術（ロボトミー）のように精神疾患の治療法として外科的手法が用いられることがあった（精神外科）。我が国において日本精神神経学会が精神外科を否定する決議を行ったのは本アルバムがリリースされた2年後の1975年になってからである。続いて閉鎖病棟に対する批判的意見と思われる部分もある。
