After a while, the rules of residency become the rules of life. Rule #1: Always keep score. Rule #2: Do whatever you can to outsmart the other guy. Rule #3: Don't make friends with the enemy.

It's Dead Baby Bike day in Seattle the day of an annual bike race with no rules (sponsored by the Dead Baby Bar). This throws off the hospital's entire surgical schedule, filling up the surgical floor with free spirited albeit injured bike messengers, including one named Viper with spokes sticking out of his abdomen. Meredith and Alex both want him, but Viper immediately takes to Alex's handling of the situation Alex just yanks out the spokes and lets him get back to the race, leaving Meredith to suture and do the paperwork. Meredith reluctantly discharges the charismatic Viper, but not before he kisses her in front of a jealous Derek who claims not to be jealous.

The race isn't all just cuts and bruises. There is a casualty. Izzie and Cristina are put in charge of an unidentified visitor to Seattle (has no wallet, just a hotel key card), who was hit by a car when jumped out of the way of the racers. The patient is put on the clock if there's no sign of brain activity in 6 hours, he's to be declared brain dead. This is a very troubling prospect for Izzie.

George gets a special assignment from Richard, attend to a close, personal friend who's been admitted with liver failure. When the male patient makes a pass at George, George becomes paranoid that others in the hospital namely his crush Meredith might think he's gay.

Should Cristina and Izzie's patient be declard brain dead, Izzie wants to make the guy's death meaningful by getting his organs donated. But that's nearly impossible when the person is a John Doe. Cristina wants his organs donated, too, but really just because she wants to be in a harvest. While the man's family is tracked down, though, the patient crashes, needing a blood transfusion as well as surgery to keep his body viable as a donor. Izzie and Meredith do the transfusion, but Burke has to do the surgery, and he doesn't believe in operating on patients that are already dead (the six hours are almost up). So he declines. And without Burke's surgery, it won't be much longer before the heart fails again.

Derek lets Meredith in on a little secret: "You want Burke to do the surgery, get Richard on board first." And they do, by going to George and getting him to tell Richard that they may have a liver for his friend. It works, but not without raising Burke's ire. The surgery is done, the family is tracked down, and now all they need to do is get the patient's wife to agree to organ donation. It's a job that Cristina can't really handle, as observed by Bailey (who shows she can be as thoughtful to patients and family as she can be tough on her interns).

Viper returns, but not for another kiss with Meredith. He's sustained a life threatening injury and Bailey and Meredith have to save him. The prospect of the harvest, though able to give the man's death more meaning, still isn't satisfying to Izzie. She really wanted the patient to wake up, to live. And Burke understands where she's coming from. The bittersweet harvest is done, giving Richard's friend a new liver, and when Izzie comes in to sew up the brain dead guy (who's now just dead guy) so he can be presentable to his family, Cristina sticks around to help, seeing the patient as a human being.

Ross follows Weaver as she rushes baby Josh up to the pediatric intensive care unit. They encounter Dr. Babcock (DAVID BRISBIN), a pediatric anesthesiologist, who continues Ross' rapid detox procedure on the baby--much to Weaver's surprise. Later, the treatment proves successful. But even though Ross saved the little boy, he may be terminated from his job.
　Rossは赤ちゃんJoshを小児集中医療室に急いで運ぶWeaverを追う。そこには、小児麻酔医Dr. Babcockがいて、Rossの超急速解毒を継続する。Weaverは驚く。その後、処置は成功したことが分かる。しかし、Rossが幼い子供を助けたとしても、クビになるかもしれない。

Romano informs Corday--via letter--that he won't sponsor her next year. Reese's day-care worker thinks Reese might have a hearing problem. Carter suspects Rosher stole a missing vial containing a pain killer. Rosher tells Del Amico that if he recommends an e.r. pediatric department, he might like to stay and manage it.

Weaver asks Anspaugh to appoint her as permanent chief of emergency medicine, and he informs her that there will be a national search for the best candidate, per county policy. An insurance company refuses to allow a 50-year-old man, Mr. Wass (ROGER ROBINSON), to get the anticoagulation medicine Weaver prescribes. A furious Weaver vocally resigns as acting chief.

A wife (CARRIE SNODGRESS - "Diary of a Mad Housewife") must decide whether to keep her terminally ill husband in a coma or order surgery that will wake him up but not save his life. To Hathaway's chagrin, a psychiatric resident interviews and releases Victor Nable (MICHAEL RISPOLI - "While You Were Sleeping"), a 40-year-old man brought to the e.r. with superficial lacerations on his wrist. Later, when a bloodied Nable returns to the e.r., Hathaway discovers Nable's wife and two children shot but all still alive. The entire e.r. crew works feverishly to save the family.
　夫をこん睡状態のままにするか、手術をしていったん目を覚まさせるかの選択を妻に迫る。どちらにしろ、命を救うことは不可能だ。精神科のレジデントのインタビューで、手首の浅い裂傷でERに運ばれて来た40歳の男性Victor Nableを退院させたのを悔やむ。その後、血まみれになってERに戻ってくる。Hathawayは銃撃されたNableの妻と二人の子供を見つける。ERの職員全員が一生懸命に家族を助ける。

Jeanie fears she might be getting ill, but tests prove she's fine. Benton and Corday believe that Romano saw them kissing in the locker room. Romano implies to Corday that he will not sponsor her next year. Benton confronts Romano about Benton's relationship with Corday, something Romano was not aware of.

Dr. Max Rosher (JAMES LeGROS - "Born on the Fourth of July") performs a feasibility study on setting up a pediatric unit in the e.r. Formerly addicted to pain-killers, Rosher is Del Amico's ex-attending physician from Philadelphia. He's also rekindled his romance with her and is staying at Del Amico's home. Ross hopes that Rosher will recommend creating a pediatric attending position for Ross. However, Rosher also appears interested in the job.
　Dr. Max RosherはERに小児科専門医を用意すべきか検討に入る。以前、鎮痛剤依存だったRosherはPhiladelphiaではDel Amicoの主治医だった。彼は彼女との恋が再燃し、Del Amicoの家に泊まっている。RossはRosherが小児科専門医を作ってくれるのに期待するが、Rosherは仕事にも興味を持っているようだ。

Tina Marie Chambliss (SWOOSIE KURTZ - "Sisters"), a televangelist from the Christian Cable Network, enters the e.r. with complications due to a recent face-lift. The tests Del Amico performs indicate the diabetic Tina Marie has a kidney problem. Tina Marie and her husband/business partner use the bad news to telecast a live broadcast so that their fans can pray for Tina Marie and, more importantly, send money for her recovery.
　Christian Cable Networkのテレビ伝道師、Tina Marie Chamblissは最近した美容整形のせいで合併症を起こし、ERに運ばれてきた。Del Amicoの検査で、Tina Marieの腎臓病が糖尿病を誘発したことがわかる。Tina Marieと彼女の夫でビジネスパートナーの二人は、悪いニュースをテレビの生放送で流す。ファンがTina Marieの回復を祈り、寄付金を集めることが本当の目的だ。

Meanwhile, the 6-month-old methadone-addicted Josh will be returned to his mother, who steals his drugs. An angry Ross tells the child services employee that Josh must stay in the hospital because he might have pneumonia, which is not true. Ross begins an experimental, ultrarapid detox that will take 12 to 18 hours. He can't admit Josh because he does not have the mother's consent so Ross, with Hathaway's assistance, performs the procedure surreptitiously. Greene sees them working on the unregistered patient. A furious Weaver and Greene chastise Ross for disobeying hospital rules. Ross and Hathaway could be terminated from their jobs.
　一方、生後6ヶ月で麻薬中毒のJoshを、子供のドラッグを盗んでいた母に戻ることになっている。怒ったRossは、児童局に、肺炎に罹ったフリをして、そのために病院に強引に引き止めると話す。Rossは12～18時間かかる超急速解毒を施す。母の承諾を得ていないため、許されない行為だったが、Hathawayの協力で、内密に施すことにする。Greeneは未登録の患者に治療しているのを見つける。激怒したWeaverとGreeneは病院の規則を破ったことを厳しく非難する。RossとHathawayは退職に追い込まれるかもしれない。
