If you?ve developed a solid bump at the base of your big toe along with pain and swelling, it?s possible that you have a bunion. According to the American Podiatric Medical Association (APMA) A bunion is an enlargement of the joint at the base of the big toe-the metatarsophalangeal (MTP) joint, that forms when the bone or tissue at the big toe joint moves out of place. This forces the toe to bend toward the others, causing an often painful lump of bone on the foot. Since this joint carries a lot of the body?s weight while walking, bunions can cause extreme pain if left untreated. The MTP joint itself may become stiff and sore, making even the wearing of shoes difficult or impossible. Bunions, from the Latin ?bunio,? meaning enlargement, can also occur on the outside of the foot along the little toe, where it is called a ?bunionette? or ?tailor?s bunion.? Causes
You may get bunions if the way your foot is shaped puts too much pressure on your big toe joint. Because bunions can run in families, some experts believe that the inherited shape of the foot makes some people more likely to get them. Your foot rolls inward too much when you walk. A moderate amount of inward roll, or pronation, is normal. But damage and injury can happen with too much pronation. You have flat feet. You often wear shoes that are too tight. All of these may put pressure on the big toe joint. Over time, the constant pressure forces the big toe out of alignment, bending it toward the other toes.
Symptoms
Alteration in alignment of the first toe. Pain in the 1st toe joint with movement. Restriction in range of demi pointe. Inflammation of the 1st toe joint. Rotation of the big toe so that the nail no longer faces upwards. Occasionally bruising of the toe nail occurs.
Diagnosis
Before examining your foot, the doctor will ask you about the types of shoes you wear and how often you wear them. He or she also will ask if anyone else in your family has had bunions or if you have had any previous injury to the foot. In most cases, your doctor can diagnose a bunion just by examining your foot. During this exam, you will be asked to move your big toe up and down to see if you can move it as much as you should be able to. The doctor also will look for signs of redness and swelling and ask if the area is painful. Your doctor may want to order X-rays of the foot to check for other causes of pain, to determine whether there is significant arthritis and to see if the bones are aligned properly.
Non Surgical Treatment
A bunion treatment must address the underlying cause of the deformity, not just the bump (bunion) itself but also the functions of the foot. The up and down motion of the longitudinal arches in the foot. The sideways motion of the transverse arch. Bunion aids effectively treat this underlying foot function while straightening the big toe because the mid-foot strap stabilizes the longitudinal arches and transverse arch. The toe strap gradually and gently pulls the big toe away from the second toe. The metatarsal pad helps align the transverse arch. The hinged splint enables the big toe to flex while walking and adapts to the contour of the foot, especially around the inflamed area of the joint.
Surgical Treatment
In 2010, the National Institute for Health and Care Excellence (NICE) published guidance about a minimally invasive surgical procedure to treat bunions. The aim of the procedure is to repair the tilting of the big toe. The technique can be carried out under a local anaesthetic or a general anaesthetic, using X-rays or an endoscope for guidance. The type of endoscope used will be a long, thin, rigid tube with a light source and video camera at one end. One or more incisions will be made near the big toe so that bone-cutting instruments can be inserted. These will be used to remove the bunion and to divide one or more bones located at the front of the foot. Wires, screws or plates will be used to keep the divided bones in place. After the procedure, you may need to wear a plaster cast or dressing to keep your foot in the correct position until the bones have healed. You may be given a special surgical shoe that enables you to walk on your heel. As the procedure is relatively new, there's little in the way of reliable evidence regarding its safety or effectiveness.
